Background: Removal of dislocated lower third molar into the submandible region is quite rare. Attention should be taken to avoid mandible fracture. Methods: This is a case report of a 56-year-old patient had an ectopic inverted position lower left third molar in submandible region, suffered from a osteomyelities at the left jaw. Removal of the molar through extraoral approach was chosen avoiding unnecessary scar formation, the tooth was separated and removed. Results: Removal through extraoral approach had been done with good result. Summary: Sequesterectomy with extraoral approach could be done depending location of the teeth to minimize unnecessary scar formation.</p
