Introduction: Gastric cancer (GC) is the fifth leading cause of cancer in Brazil. Despite the poor prognosis, palliative chemotherapy is the standard treatment for patients with metastatic disease. However, there is still controversy about the best scheme. Objectives: To evaluate the overall survival (OS), response rate (RR) and toxicity in patients with metastatic GC undergoing chemotherapy, to compare the cost-effectiveness of regimens, and assess the prognostic value of tumor expression of HER2 in patients with no specific treatment for this condition. Methods: This is a retrospective observational study, including patients treated in the last four years in UNIFESP oncology sector. Results: 47 patients were included. In the first-line setting: 29 received chemotherapy based on fluoropyrimidine plus platinum (FP) - mean of 59 years (24-88), 34% had ECOG 2 and received an average of 5.7 cycles; 16 were treated with a triple regimen (EOX) - mean of 47 years (27-71), 19% had ECOG 2 and completed 5.4 cycles on average. The remaining two patients were treated with single agent. The median OS of the entire population was 13.8 months. RR was 64% for EOX and 38% for FP (P = 0.25). The median PFS was 9.5 and 5.6 months, respectively (HR 0.85; 95% CI: 0.41 to 1.74). Grade 3 or 4 adverse events were observed in 56% and 28% of patients, respectively (P = 0.64). The cost-effectiveness of treatments was similar (approximately R$ 23,000 for each additional year of survival). However, the ratio reduced by 50% when EOX was administered to patients with ECOG 0 or 1 under 65 years of age. Samples from 35 individuals were available for evaluation of the expression of HER2 and 16% were positive. Favorable histology (intestinal) (HR 0.34, 95%CI: 0.13 to 0.91), achieving second-line treatment (HR 0.25; 95%CI: 0.11 to 0.58) and response to the treatment (HR 0.23; 95%CI: 0.12 to 0.47) were prognostic factors associated with better OS. The expression of HER2 had a trend toward worse prognosis (HR 1.52; 95%CI: 0.50 to 4.66). Conclusion: Patients treated in our hospital showed PFS and OS compatible with the literature. There was no difference between the protocols with two or three drugs. However, the cost-effectiveness of the treatment improved with the regimen-choice based on clinical aspects. Apesar do prognóstico desfavorável, quimioterapia paliativa é o tratamento padrão para pacientes com doença metastática. Entretanto, há controvérsias sobre qual o melhor esquema. Objetivos: Avaliar a sobrevida global (SG), a taxa de resposta (TR) e a toxicidade em pacientes com CG metastático submetidos à quimioterapia, comparar a relação custo-efetividade dos esquemas e avaliar o valor prognóstico da expressão tumoral de HER2 em pacientes sem tratamento específico para essa condição. Métodos: É um estudo retrospectivo observacional, incluindo pacientes tratados entre 2011 e 2015 no setor de Oncologia da UNIFESP. Resultados: 47 pacientes foram incluídos. Na primeira linha de quimioterapia: 29 receberam esquema duplo baseado em fluoropirimidina e platina (FP), média de 59 anos (24-88), 34% apresentava ECOG 2 e receberam em média 5,7 ciclos; 16 foram tratados com esquema triplo (EOX), média de 47 anos (27-71), 19% apresentava ECOG 2 e completaram em média 5,4 ciclos. Os dois pacientes restantes foram tratados com monoterapia. A mediana da SG de todos os incluídos foi 13,8 meses. A TR foi 64% para EOX e 38% para FP (P=0,25). A mediana da SLP foi respectivamente 9,5 e 5,6 meses (HR 0,85; IC95%: 0,41 - 1,74). Eventos adversos grau 3 ou 4 foram observados em 56% e 28% dos pacientes, respectivamente (P = 0,64). A relação custo-efetividade dos tratamentos foi semelhante (cerca de R$ 23.000 para cada ano adicional de sobrevida). Entretanto, essa relação reduziu em 50% quando o tratamento EOX foi administrado para pacientes com ECOG 0 ou 1 com idade inferior a 65 anos. Amostras de 35 participantes estavam disponíveis para avaliação da expressão do HER2 e 16% eram positivas. Histologia favorável (intestinal) (HR 0,34; IC95%: 0,13 ? 0,91), receber o tratamento de segunda linha (HR 0.25; IC95%: 0,11 - 0,58) e apresentar resposta ao tratamento (HR 0,23; IC95%: 0,12 ? 0,47) foram fatores prognósticos associados com SG prolongada. A expressão do HER2 apresentou tendência de relação com pior prognóstico (HR 1,52; IC95%: 0,50 ? 4,66). Conclusão: Os pacientes tratados em nosso hospital apresentaram SLP e SG compatíveis com a literatura. Não houve diferença entre os protocolos com dois ou três medicamentos. Contudo, a relação custo-efetividade do tratamento melhorou com a escolha baseada em aspectos clínicos. Tratamento de segunda linha deve ser considerado sempre que possível.
