Tratamiento por técnicas endovasculares de los aneurismas cerebrales. Primeras experiencias en Venezuela.

Introduction: Rupture of an aneurysm is the most frequent cause of subarachnoid hemorrhage. Surgical clipping being the standard treatment for ruptured cerebral aneurysms, with endovascular embolization as a new alternative. Objective: To review the preliminary experience at the Hospital Universitario de Caracas in the endovascular treatment of ruptured cerebral aneurysms, as compared to the standard surgical clipping. Methods: between august 2000 and march 2001 20 ruptured aneurysms were embolized with Guglielmi detachable coils, comparing with 20 cerebral aneurysms surgically treated. Results: Technical success of 100% was obtained in both groups. Presence of complications was 15% in both groups. No significant differences about age, sex, consultation motive or antecedents were found. The permanence time in the intensive care unit and in general care showed a significant difference (p< 0.5), being shorter for the embolized cases. Conclusion: the endovascular embolization is an excellent alternative for ruptured cerebral aneurysms, with identical morbimortality rate to that reported for surgical treatments.Introducción: La rotura de un aneurisma es la causa más frecuente de hemorragia subaracnoidea. El clipaje quirúrgico es el tratamiento estándar para los aneurismas cerebrales rotos, y la embolización endovascular es una nueva alternativa. Objetivo: presentar la experiencia inicial del Hospital Universitario de Caracas en el tratamiento endovascular de los aneurismas cerebrales rotos, comparándola con el clipaje quirúrgico. Métodos: Entre agosto 2000 y marzo 2001 se embolizaron 20 aneurismas cerebrales rotos con resortes tipo Gluglielmi, comparándolos con 20 aneurismas intervenidos quirúrgicamente. Resultados: hubo 15% de complicaciones, con éxito técnico de 100% en ambos grupos. No hubo diferencia significativa para edad, sexo, motivo de consulta o antecedentes. El tiempo de permanencia en UTI y en cuidados generales presentó diferencia significativa (p< 0.5), siendo menores para los embolizados. Conclusión: la embolización endovascular es una excelente alternativa terapéutica para los aneurismas cerebrales rotos, con índice de morbimortalidad igual al reportado con el clipaje quirúrgico
