The aging brain : vulnerability of the frontal lobes

L’invecchiamento è definibile come un processo irreversibile che caratterizza gli organismi viventi, il quale coinvolge anche il cervello. E’ possibile distinguere tra invecchiamento “sano” e “patologico”: nel primo caso si fa riferimento al deterioramento delle aree neurali causato dall’aumentare dell’età, mentre nel secondo caso si fa riferimento ai casi, come quello della demenza, nei quali gli insulti del tessuto neurale sono causati da una malattia e portano a conseguenze più gravi. E’ stata avanzata l’ipotesi che la suscettibilità all’invecchiamento sano possa variare, non a livello individuale, ma anche a livello delle aree cerebrali interessate. Secondo l’ipotesi dell’invecchiamento frontale (Dempster 1992, West 1996), i lobi frontali sono le aree cerebrali maggiormente sensibili al deterioramento neurale e cognitivo. Coerentemente, i dati neuroanatomici dimostrano che le lesioni e la riduzione del volume della sostanza grigia e bianca sono più accentuate a livello delle aree frontali rispetto ad altre aree del cervello. Queste modificazioni neurali causano deficit cognitivi specifici, evidenziabili tramite test neuropsicologici mirati allo studio delle funzioni frontali. Sono stati proposti numerosi modelli capaci di spiegare, da prospettive diverse, in che modo le aree frontali vanno incontro a modificazioni nel corso dell’invecchiamento assieme ai meccanismi di riorganizzazione funzionale che utilizzano per far fronte ad esso. In particolare, ciò avviene mediante meccanismi di compensazione e di creazione di scaffolding, che consentono ai soggetti anziani di mantenere un livello di performance adeguato. La vulnerabilità all’invecchiamento e la capacità di far fronte ad esso sono soggette a variabilità in quanto esistono fattori di protezione che agiscono contro il deterioramento cognitivo, tra i quali l’istruzione e la stimolazione cognitiva. A tal proposito, sono stati creati dei programmi capaci di promuovere un miglioramento delle performance cognitive, come il progetto Synapse. Di conseguenza, comprendere in che modo l’invecchiamento normale influisce sulle funzioni frontali è di grande importanza per porre attenzione sulle abilità cognitive che vanno incontro a deterioramento e quelle preservate, così da potere promuovere la neuroplasticità e un adeguato mantenimento delle funzioni cognitive, assicurando un invecchiamento di successo.Aging can be defined as an irreversible process that characterizes living organisms, which also involves the brain. It is possible to distinguish between "healthy" and "pathological" aging: in the first case we refer to the deterioration of the neural areas caused by increasing age, while in the second case we refer to cases, such as dementia, in which the insults of neural tissue are caused by an illness and lead to more serious consequences. It has been advanced the hypothesis that the susceptibility to healthy aging may vary, not only individually, but also at the level of the affected brain areas. According to the hypothesis of frontal aging (Dempster 1992, West 1996), the frontal lobes are the brain areas most sensitive to neural and cognitive deterioration. Coherently, neuroanatomical data show that indeed the lesions and the reduction of the volume of the gray and white matter are more accentuated at the level of the frontal areas than in other areas of the brain. These neural modifications cause specific cognitive deficits, evidenced by neuropsychological tests aimed at the study of frontal functions. Several models have been proposed, trying to explain how frontal areas undergo changes in the course of aging and the functional reorganization mechanisms used to cope with it. In particular, this occurs through compensation and scaffolding creation mechanisms, which allow elderly subjects to maintain an adequate level of performance. Vulnerability to aging and the ability to cope with it are subject to variability as there are protective factors that act against cognitive impairment, including education and cognitive stimulation. In this regard, programs have been created with the purpose of promoting an improvement in cognitive performance, such as the Synapse project. Consequently, understanding how normal aging affects frontal functions has great importance because it allows to pay attention to the cognitive abilities that undergo deterioration and those preserved, so as to be able to promote neuroplasticity and an adequate maintenance of cognitive functions, ensuring a successful aging
