La psicoprofilaxis obstétrica y estimulación perinatal temprana una alternativa terapéutica grupal para la disminución sintomatológica depresiva en madres adolescentes

The group of adolescents constitutes an extremely important field of study, because it represents the age at which the active sex life begins. The World Health Organization (WHO) defines adolescence as the stage between the ages of ten and nineteen. 10% of births annually in the world are of teenage mothers, in Mexico it is a priority issue of sexual and reproductive health. The population of pregnant adolescents has become a health problem. Mexico ranks first in teenage pregnancy throughout Latin America". Therefore, the risk of developing depression is very high, considering that depression is classified as 21st century disease the OMS Objective: To understand depression in adolescent women and their relationship to psychoprophylaxis and perinatal stimulation have an effect on depressive symptomatology in a sample of pregnant adolescents who come to the rural hospital IMSS-Prospera No. 44 of Zacatipan, Tamazunchale S.L.P. Methodology: Quantitative study, quasi-experimental, in pregnant adolescents population (12-19 years), data collection through sociodemographic data and obstetric gynecology, screening interview for early detection of depression in pregnancy control consultations, application of the instrument of the Edinburgh Depression Scale, in preintervention and postintervention, which measures depressive symptoms, the application of educational intervention through sessions of obstetric psycho-prophylaxis techniques and early perinatal stimulation and the survey prepared by the research team to observe the impact of the methodology of psychoprophylaxis and perinatal stimulation in depressive symptomatology. For the analysis of the data, the Student t test and the statistical package SPSS version 18 were applied to determine the correlation of the impact of psychoprophylaxis on depressive symptomatology in adolescents. Results: Group formed by adolescents pregnant between 15 and 19 years old, single marital status and free union, dedicated to housework, with suspension of educational activities. Completed secondary education level and coming from urban area. Gynecological obstetric history: first pregnancy, 55.6% in second trimester, low obstetric risk predominates, no obstetric complications in prenatal control. At follow-up, 77.8% presented normal delivery. They present risk factors such as maternal anxiety, lack of support from the couple and attending with an unwanted pregnancy. In their participation in sessions of psychoprophylaxis describe benefits such as improvement in their mood and the relationship with their unborn baby. In relation to the measurement of the level of depression pre- and post-intervention educational, through the application of the Edinburgh Prenatal Depression Scale; In the preintervention period, an average of 10.33 with suspected depression and a 4.83 without suspicion are reported in the group of participating pregnant adolescents, after which the educational intervention on psychoprophylaxis and perinatal stimulation is implemented. Depression for its final measurement, obtaining an average of 1.1 with suspected depression, presented a difference of 1.3 between the preintervention and postintervention measurements with a t = 10 and a p = .000 finding a statistically significant difference. Conclusions: Obstetric psychoprophylaxis and early perinatal stimulation can be used to prevent and reduce depression in pregnant adolescents and the Edinburgh test is a useful tool to observe the behavior of depressive symptomatology during the gestation process. The timely detection of risk factors from prenatal consultation is a useful tool for the promotion of mental health in this vulnerable population group.El grupo de los adolescentes constituye un campo de estudio sumamente importante, porque representa la edad en que se inicia la vida sexual activa. La Organización Mundial de la Salud (OMS) define a la adolescencia como la etapa comprendida entre los diez y los diecinueve años. El 10% de los nacimientos anualmente en el mundo son de madres adolescentes, en México es un tema prioritario de salud sexual y reproductiva. La población de adolescentes embarazadas se ha vuelto un problema de salud. México ocupa el primer lugar de embarazo en adolescentes en toda América Latina”1 por lo que el riesgo de presentar depresión es muy alto, considerando además que la depresión es catalogada como la enfermedad del siglo XXI según la OMS. Comprender la depresión en la mujer adolescente y su relación con la psicoprofilaxis y estimulación perinatal tiene un efecto en la sintomatología depresiva en una muestra de adolescentes embarazadas que acuden al hospital rural IMSS-Prospera No. 44 de Zacatipan, Tamazunchale S.L.P. Estudio cuantitativo, cuasi experimental, en población adolescentes embarazadas (12-19 años), recolección de datos mediante cedula de datos sociodemográficos y gineco obstétricos, la entrevista de tamizaje de detección precoz de la depresión en consultas de control del embarazo, la aplicación del instrumento de Escala de depresión de Edimburgo, en preintervencion y postintervencion, que mide síntomas depresivos, la aplicación de intervención educativa mediante sesiones de técnicas de psicoprofilaxis obstétrica y de estimulación perinatal temprana y la encuesta elaborada por el equipo investigador para poder observar el impacto de la metodología de la psicoprofilaxis y estimulación perinatal en la sintomatología depresiva. Para el análisis de los datos se aplicó la prueba t de Student y el paquete estadístico SPSS versión 18, para determinar la correlación del impacto de la psicoprofilaxis en la sintomatología depresiva en las adolescentes. Grupo formado por adolescentes embarazadas entre 12 a 19 años, estado civil soltera y unión libre, dedicadas a labores del hogar, con suspensión de actividades educativas. Nivel educativo secundaria terminada y provenientes de área urbana. Antecedentes ginecoobstetricos: cursado primer embarazo, 55.6% cursa segundo trimestre, predomina riesgo obstétrico bajo, sin complicaciones obstétricas en control prenatal. En el seguimiento 77.8% presentó parto normal. Presentan factores de riesgo como ansiedad materna, falta de apoyo de la pareja y cursar con un embarazo no deseado. En su participación en sesiones de psicoprofilaxis describen beneficios como mejoría en su estado de ánimo y la relación con su bebe en gestación, de acuerdo al análisis estadístico realizado por la encuesta elaborada por el equipo investigador. En relación a medición del nivel de depresión pre y postintervencion educativa, mediante la aplicación de la Escala de depresión prenatal de Edimburgo; en el periodo de preintervencion se reporta un promedio de 10.33 con sospecha de depresión y un 4.83 sin sospecha, en el grupo de adolescentes embarazadas participantes, posterior se implementa la intervención educativa sobre psicoprofilaxis y estimulación perinatal, realizando en periodo postintervencion nueva aplicación de escala de depresión para su medición final obteniendo un promedio de 1.1 con sospecha de depresión, presentado una diferencia de 1.3 entre la medición preintervencion y postintervencion con una t=10 y una p=.000 encontrando una diferencia estadísticamente significativa
