Prévalence et facteurs de risque de le reprise du tabac en post-partum

Introduction : For women, pregnancy is a favourable time to quit smoking, however many women fall back in after giving birth.Method : The main purpose of the study was to measure the frequency of the relapse into tobacco use for women, three and six months after giving birth. The second purpose was to identify the risk or protective factors of the relapse for these women.In order to do that, a prospective observational study was led many times with 162 women. They were recruted after giving birth within a type III maternity facility. A first survey were given to them at the maternity facility, then two others were sent via email three and six months postpartum.Results : Three months postpartum, 43,52% of the women started smoking again, then six months postpartum, it is 46,27% of them that felt back into tobacco use. The majority of women who started smoking again have a minimum of 1 cigarette a day. Breastfeeding is identified as a protective factor of a relapse. The assessment of the craving through the FTCQ-12 survey is a predictive indicator for the abstention's failure. Women with a partner who stopped smoking during the three first postpartum months had significally a reduced risk to fall back into smoking.Discussion : The consistent reports show that the rates of recidivism in tobacco use match with the datas found in reports. Some risk factors found in other studies haven't been highlighted in this study as the number of interviews were too low. They would earn to get large-scale studies in order to evaluate their influence.Conclusion : The use of the FTCQ-12 score to identify women with a risk of a relapse maybe be use as a common practice. Promoting breast feeding and the involvment of the partner have to be considerated as a preventive measure of the recidivism in tobacco use.Introduction : Chez les femmes, la grossesse est un moment propice au sevrage tabagique mais une grande partie d’entre elles recommencent à fumer après l’accouchement.Méthode : L’objectif principal de l’étude était de mesurer la prévalence de la reprise du tabac chez les femmes à trois et six mois du post-partum. L’objectif secondaire était d’identifier les facteurs de risque ou protecteurs de la reprise du tabagisme chez ces femmes. Pour ce faire, 162 femmes ont été recrutées après leur accouchement au sein d’une maternité de type III. Un premier questionnaire leur a été remis à la maternité puis deux autres questionnaires, à trois et six moi du post-partum, leur ont été envoyé par courriel.Résultats : A trois mois du post-partum 43.52% des femmes recommençaient à fumer, et à six mois du post-partum elles étaient 46.27%. La majorité des reprises correspondait à des rechutes (consommation quotidienne de cigarette). L’allaitement maternel a été identifié comme facteur protecteur de la rechute. L’évaluation du craving par le questionnaire FTCQ-12 est un indicateur prédictif de l’échec de l’abstention. Les femmes dont le/la conjoint.e initialement fumeur effectuait un sevrage tabagique dans les trois premiers mois du post-partum avaient significativement moins de risque de reprendre le tabagisme. Les femmes fumeuses en post-partum avaient une faible motivation à vouloir arrêter de fumer.Discussion : Les taux de récidive tabagique retrouvés sont concordants avec les données retrouvées dans les études. Certains facteurs identifiés comme à risque de reprise dans la littérature mériteraient des études à plus grande échelle pour évaluer leur influence.Conclusion : L’utilisation du score FTCQ-12 pour identifier les femmes à risque de rechute pourrait être utilisé dans la pratique courante. La promotion de l’allaitement maternel ainsi que l’implication du conjoint doivent être effectués comme mesure de prévention de la récidive tabagique
