Hormon tedavisi alan premenapozal ve postmenapozal kadınların mammogrofi bulgularındaki değişiklikler

These are the treatment of the vasomotor symptoms, the prevention of the urogenital atrophy and the prevention of the osteoporosis. For all these reasons, the benefit of the hormonal therapy which is regulated for the patient is certain. Although, the discussions about it to incrase the breast canser stil continues. Mammography is a gold standart procedure in the early diagnosis of the breast canser. It deareases the mortality dependent on the breast cancer in the 50 years old or older women. However mammography is the most sensitive procedure in the diagnosis of the breast cancer, its sensitivity is less than 85-90%. The main reason for this is, the tendency of the dense breast tissue in hiding the lesion lying underneath. The interpretation of the mammographies is highly correspondent to the high risk of the breast cancer. In different studies, hormonal therapy is related to the increase in the density of the breast in 25% of the overall cases. This condition is important in the follow up the patients taking hormone therapy. Over goal in this study is to observe the changes in the mammogaphic findings of the women taking hormone therapy in the premenopausal and postmenopausal periods. Due to this aim, 630 cases, attending to the menopause clins of the gynecology anda obstetrics department of the Medicine Faculty of the Çukurova University betwen the year January 2000 and December 2005, are taken into the study. 441 patients were taking hormone therapy and 189 patients weren't taking any therapy and this is control group. The mammographies of the patients before and after the hormone therapy were discussed retrospectively according to the changes in the pattern, lesion and density. At the end of the study tibolon and conjugated oral estrogen plus+MPA taking group shoud a 3% increase in the liposclerosan or sclerozan (dens) breast pattern, there is showed is a 4% increase among cyclic therapy taking patients. Only oral estrogen taking group didn't show any change in the pattern. Conversely, there was an increase in the lipomatous breast pattern among no therapy taking group andTüm dünyada hormon tedavisi üç ana endikasyon için onay almıştır. Bunlar vasomotor semptomların tedavisi, ürogenital atrofinin önlenmesi ve osteoporozun önlenmesidir. Tüm bu nedenlerden dolayı kiºiye özgü düzenlenmiº bir hormon tedavisinin sağlayacağı yarar tartışmasızdır. Bununla birlikte meme kanseri riskini arttırdığında dair tartışmalar halen sürmektedir. Mammografi, meme kanserinin erken tanısında altın standart olan bir yöntemdir. Elli yaº üzerindeki kadınlarda meme kanserine bağlı mortaliteyi belirgin oranda azaltmaktadır. Her ne kadar mammografi meme lezyonlarının tanısında en duyarlı yöntem olsa da duyarlılığı % 85-90'nın üzerine çıkmamaktadır. Bunun en önemli nedeni yoğun meme dokusunun, altında yatan lezyonu gizleme eğilimidir. Mammografilerin kullanımı kolaydır ve mammografideki dansite artışı, artmış meme kanseri riski ile yüksek oranda iliºkilidir. Hormon tedavisinin değiºik çalışmalarda % 25 olguda meme dansitesinde artış ile iliºkili olduğu söylenmektedir. Bu durum hormon tedavisi altındaki hastaların takibinde memenin değerlendirmesini önemli kılmaktadır. Bu çalışmadaki amacımız, hormon tedavisi alan perimenopoz ve postmenopozal dönemdeki kadınların mammografi bulgularındaki değiºiklikleri incelemek idi. Bu amaçla 2000-Aralık 2005 tarihleri arasında Ç.Ü. Tıp Fakültesi Kadın Hastalıkları ve Doğum AD Menapoz Kliniğine baºvuran, toplam 630 olgu çalışmaya alındı. Hastaların 441'i hormon tedavisi alan hastalardan oluºuyordu. 189 hasta herhangi bir tedavi almayan kontrol grubu idi. Çalışmaya alınan olguların hormon tedavisi öncesi ve sonrası çekilmiº mamografileri retrospektif olarak değerlendirildi. Mamografiler, pattern değiºikliği, lezyon değiºikliği ve dansite değiºikliği açısından değerlendirildi. Çalışma sonucunda tibolon ve konjuge oral östrojen+MPA alan grupta % 3 oranında LSK/SK (Dens) meme patterni oranı artarken siklik tedavi alanlarda % 4 oranında artış oldu. Sadece oral östrojen alan grupta pattern değiºikliği izlenmedi. Hormon tedavisi almayan grupta ve Transdermal östrojen alan grupta ise hormon tedavisi sonrasında tam tersi lipomatö meme patterni artmış olarak saptandı. Hormon tedavisiBu çalışma Ç.Ü.
