The Diagnostic Value of Procalcitonin, C-Reactive Protein and Interleukin-6 in Early Recognition of Severe Sepsis.

TEZ9323Tez (Uzmanlık) -- Çukurova Üniversitesi, Adana, 2014.Kaynakça (s. 82-94) var.x, 99 s. : tablo ; 29 cm.Amaç: Sepsis, ağır sepsis, septik şok, Sistemik İnflamatuar Yanıt Sendromu ve bakteriyel enfeksiyon tanısı alan hastalarda yapılan bu çalışmada ağır sepsisin erken tanısında plazma C-reaktif protein, Prokalsitonin ve Interlökin-6 düzeylerinin erken tanıdaki yerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmaya 100 hasta alınmıştır. Bu hastalar kliniğine göre 26 çocuk sepsis (Grup I), 17 çocuk ağır sepsis (Grup II), 22 çocuk septik şok (Grup III), 9 çocuk SIRS (Grup IV) ve 14 çocuk bakteriyel enfeksiyon (Grup V) ve 12 çocuk kontrol (Grup VI) grubunu oluşturdu. Hastaların yaş, cinsiyet, başvuru şikayetleri, fizik muayene bulguları, tam kan sayımı, akciğer grafisi bulguları, kültür bulguları, uygulanan tedavi biçimleri ve tedavi sonrası nihai durumları kaydedildi. Hastaların serum örneklerinden plazma PCT, CRP ve IL-6 düzeylerine bakıldı. Bulgular: Tüm hasta gruplarında ortalama serum PCT düzeyleri kontrol grubuna göre yüksek bulundu. CRP değerleri bakımından, gruplar arasında fark saptanmadı. Ortalama IL-6 düzeyleri kontrol grubuna göre hasta gruplarında daha yüksek bulundu. Sonuç: Prokalsitonin, İnterlökin-6 ve C-reaktif protein gibi birçok biyobelirteçlerin ağır sepsisin erken tanısında ve tedavinin takibinde kullanılabileceğini gösteren birçok çalışma mevcut olup, biz çalışmamızda hastaların erken tanısında PCT’nin CRP ve IL-6’ya göre daha güçlü bir biyobelirteç olduğu sonucuna ulaştık.Objective: In this study, conducted in patients with bacterial infection, Systemic Inflammatory Response Syndrome (SIRS), sepsis, severe sepsis and septic shock, we aimed to assess the diagnostic value of Procalcitonin, C-Reactive Protein and Interleukin-6 levels in early diagnosis. Material and Method: In this study, 100 patients were included. According to clinical diagnosis; 26 children had sepsis (Group I), 17 children had severe sepsis (Group II), 22 children had septic shock (Group III), 9 children had SIRS (Group IV) and 14 children had bacterial infection (Group V) and 12 children were included as control group (Group VI). We recorded patients’ age, gender, complaints on admission, physical examination findings, complete blood count values, pulmonary X-ray findings, culture findings, treatment modalities and conditions at the end of treatment. Plasma levels of PCT, CRP and IL-6 were obtained from patients and studied. Results: Mean PCT levels were higher in all patient groups than control group. Statistically there was no significance regarding mean CRP levels between all groups. Mean IL-6 levels were higher in all patient groups than control group. Conclusion: Studies have shown biomarkers like procalcitonin, interleukin-6 and C-reactive protein can be used in early recognition and treatment monitorization of severe sepsis. In our study, we concluded that PCT is a lot more powerful diagnostic biomarker in early diagnosis of sepsis than CRP and IL-6.Bu çalışma Ç.Ü.
