Comparison of two new different anaesthetic techniques for neonatal surgical emergencies

Background: The aim of the study was to compare the efficacy and adverse effects of morphine or remifentanil analgesia or anaesthesia in neonatal surgical emergencies. Patients and Method: After the approval of Ethics Commitee and parents, 43 patients of ASA class-II, aged 0-28 days and undergoing thoracotomy or laparotomy were taken in the study. The patients were divided in two groups, randomly. After 1 mg kg-1 ketamine induction, 30 µkg-1 single dose morphine to group I patients (n=20) and 0,25 µkg-1 min-1 remifentanil infusion to group II patients (n=23) were applied. Electrocardiography, systolic and diastolic blood pressures, peripheral oxygen saturation, end-tidal carbon dioxide and, amount of urine values were observed continually. Time to extubation, and to length of time spent in the intensive care unit were recorded in neonates. Results: Demographic values were similar in groups (p>0.05). Hemodynamic variables (HR, SAP, DAP) and Sp02 were similar in groups, intaroperatively. Extubation time in morphine group was 46.89 ± 98.2 min in remifentanil group was 19.95± 28.83 min (p>0.05) To length of time spent in the intensive care unit at morphine group was 165.1 hours and remifentanil group was 86.7 hours. Conclusion: Using single dose of morphine or remifentanil infusion as the maintenence anaesthetic in neonatesundergoing thoracotomy or laparotomy supplies enough hemodynamic stability and causes no difference between postoperative extubation times and intensive care unit stay times
