COMPUTED TOMOGRAPHIC EVALUATION OF SECONDARY ALVEOLAR BONE GRAFTING IN PATIENTS WITH CLEFT LIPAND PALATE

Amaç: Bu geriye dönük çalışmanın amacı; dudak-damak yarıklı bireylerde uygulanan ikincil alveol kemik greftinin (İAKG) uzun dönemsonuçlarını konik hüzmeli bilgisayarlı tomografi (KHBT) görüntüleri üzerinde inceleyerek bir durum tespiti yapmaktır.Gereç ve Yöntem: Farklı hekimler tarafından İAKG uygulanmış olan 35 dudak-damak yarıklı bireyin (12 çift taraflı, 23 tek taraflı) greftlemesonrası (KHBT) görüntüleri incelenmiştir. Bireylerin greftleme sırasındaki ortalama yaşları 12.3 (7-37 yaş) ve greft sonrası takip süreleri ortalama2.8 (min 1- max 5.6 yıl) yıldır. İAKG değerlendirmesi tomografi görüntülerinden alınan horizontal kesitler ve üç boyutlu görüntüler üzerindenyapılmış ve I-IV arasında derecelendirilmiştir. Yarık bölgesinin % 50'den fazlası kemik ile dolu ise (derece I ve II) greft başarılı kabul edilmiştir.Horizontal düzlemde greft bölgesinden üç kesit alınmıştır: grefte komşu diş köklerinin mine-sement birleşim bölgeleri, kök uçları ve iki kesitinortası.Bulgular: İncelenen 47 greft bölgesinin 31'inde (% 66) başarılı ve kabul edilebilir kemikleşme saptanmıştır. Bireyler yaş gruplarına göreayrılarak incelendiğinde ise daimi kaninler sürdükten sonra uygulanan greftlerde başarı oranının % 33'e düştüğü tespit edilmiştir.Sonuç: İncelenen bireylerde İAKG başarısı literatürde belirtilen oranların altındadır ve bu durum ortodontik tedavi hedeflerini olumsuzetkilemektedir.Objective: The aim of this retrospective study was to assess the long-term outcome of secondary alveolar bone grafting (SABG) in patients withcleft lip and palate (CLP) using cone beam computed tomography scansMaterial and Method: The computed tomography scans of 35 patients with CLP (12 BCLP, 23 UCLP) were evaluated. Alveolar bone graftingwas performed by different surgeons. The mean age at the time of bone grafting was 12.3 years (range, 7 to 37 years) and the mean follow-upperiod was 2.8 years (range, 1 to 5.6 years). Evaluation of SBAG was performed on horizontal CT slices and on the 3D reconstruction of the imagesby assigning score of I to IV. %50 or more bone fill within the cleft area (grade I and II) was accepted as a success. Three horizontal slices wereselected from graft area between the root surfaces of the teeth mesial and distal to the cleft: slice 1, including cemento-enamel junction on theadjacent teeth; slice 2, between slices 1 and 3; slice 3, including the root apex on the adjacent teetResults: Within the 47 graft sites, 31 (% 66) were evaluated as a successful. It was also found that the success rate reduce to % 33 when graftingperform after the eruption of the cleft -associated permanent canines.Conclusion: In the evaluated patients the success rate of SABG was lower than those of the literature and thus affects negatively the orthodontictreatment goals
