Indications for Medical Treatment and Treatment Efficiency of Tubal Ectopic Pregnancy In Our Clinic.

(bzs. rnk.), tablo ; 29 cm.Amaç: Tubal ektopik gebelik tanısı alan ve methotrexate tedavisi uygulanan hastalarda, tedavi etkinliği araştırıldı. Gereç ve Yöntem: Çalışmaya tubal ektopik gebelik nedeniyle tek doz methotrexate tedavisi protokolü almış 70 hasta dahil edildi. Hastaların tümü hemodinamik olarak stabildi. Hastaların demografik özellikler; ?-hCG değerleri, transvaginal ultrasonografi bulguları ve tedavi başarısını etkileyen faktörler incelenmiştir. Bulgular: Toplam 70 olgunun verileri analiz edilmiştir. 47 olgu (%67.14) başarıyla tedavi edilmiş, 23 olguda (%32,86) tedavi başarısız olmuştur. Olguların yaş ortalaması 30,14±5,63’tür. Olguların 45(%64.28)’ine tek doz methotrexate, 25’(%35.71) ine ise ikinci doz methotrexate uygulanmıştır. Tek doz uygulanan 45 hastanın 31 (% 68.8)’inde tedavi başarılı bulunmuştur, ikinci doz uygulanan 25 hastanın 16 (% 64)’sında tedavi başarılı olmuştur. Tek doz methotrexate alıp başarısız olan 14 hastanın 8’sine laparoskopik salpenjektomi, 6’sına laparotomi ile salpenjektomi uygulanırken, ikinci doz alıp tedavi başarısızlığı görülen 9 hastanın 3’üne laparoskopik salpenjektomi, 6’sına laparotomi ile salpenjektomi uygulanmıştır. Ektopik gebeliğin ?-hCG rezolüsyon süresi; tek doz MTX tedavisi alanlarda 22,54 ± 7,63 gün, ikinci doz MTX tedavisi alanlarda ise 29,76 ± 9,59 gün olarak hesaplanmış ve iki grup arası istatistiksel olarak anlamlı fark olduğu görülmüştür (p=0,003). Sonuç:Methotrexate tedavisi tubal ektopik gebeliğin tedavisinde etkili ve güvenli bir seçenektir. Özellikle tek doz tedavisi yüksek başarı oranı sağlamaktadır ve uygulama kolaylığına sahiptir.Objective: Effectiveness of treatment was surveyed in patients who were diagnosed as tubal ectopic pregnancy and received single dose methotrexate treatment. Materials and Methods: 70 patients who received single dose methotrexate treatment due to tubal ectopic pregnancy were included. All of the patients were hemodynamically stabile. Demographic characteristic of patients, ?-hCG values, transvaginal ultrasonographic findings and factors that effect the success of the treatment were researched. Results: Datum of totally 70 cases were analysed. 47 cases (%67.14) were successfully treated, in 23 cases (%32.86) treatment was unsuccessful. Age avarage of the cases was 30,14±5,63. In 45 of the cases (%64.28), single dose methotrexate ; in 25 of them (%35.71) second dose methotrexate was applied. 31 (%68.8) of 45 patients who received single dose was succesfully treated and 16 (%64) of 25 patients who received the second dose were successfully treated. In 8 of 14 patients who received single methotrexate that result failed and applied laparoscopiy salpengectomy; 6 of them was applied laparotomy with salpengectomy.In 3 of 9 patients who received the second dose methotrexate that result failed and applied laparoscopy salpengectomy, and 6 of them were applied laparotomy with salpengectomy. The ?-hCG resolution time of single dose methotrexate group and second dose methotrexate group was 22,54±7,63 days and 29,76±9,59 days, respectively, which was statistically significant (p=0,003). Conclusion: Methotrexate treatment is effective and safe option for treatment of tubal ectopic pregnanies. Especially single dose methotrexate treatment have high successful rates and it provides easily practise
