Sinonazal polipozisin etyopatogenezinin araştırılmasında siklooksijenaz 2, vasküler endotelyal büyüme faktörü, indüklenebilir nitrik oksit sentetaz ekspresyonlarının değerlendirilmesi ve steroid tedavisi ile ilişkisi

Most of the sinonasal polyposis formation theories are trying to explain the reason of the mucosal oedema and tissue inflamation. In this study, we aimed to detect the expression level of various chemical mediators of the inflamation such as inducible nitric oxide synthase(iNOS), cyclooxygenase 2 (COX-2) and vascular endotelial growth factor (VEGF) in the polyposis tissue, and find out the effect of the steroid therapy to these mediators. In the present study, tissue biopsies were taken from the 22 nasal polyposis patients under local anesthesia before and after the corticosteroid therapy. iNOS, COX-2 and VEGF protein expression in the spesimens was assessed according to the intensity of immunohistochemical staining and histomorphological parameters, oedema, lymphocytic and eosinophilic cell infiltration were detected semi quantitatively. Polyp size, nasal symptoms, sense of smell, and headache or facial pain were assessed by an established scoring system before and after the therapy. This study revealed that there is statistically significant decrease in iNOS and VEGF protein expression level before and after the corticosteroid therapy (p=0.023, p=0.037). However, the change in COX-2 expression level was not statistically significant (p=0.307). Also, there was no statistical difference in the tissue eodema, eosinophilia and lymphoid infiltration, but was statistically significant decrease in the nasal polyposis size and in the subjective symptoms of the patients (p>0.05). In conclusion, we found out that more studies are needed to investigate the mediators that are responsible at sinonasal polyposis etiology and the effect of corticosteroid therapy.Sinonazal poliposis etyopatogenezinde öne sürülen teorilerin çoğu, polip oluşumuna yol açan temel patolojinin mukozal ödem olduğunu kabul ederek, bu ödemin nedenlerini açıklamaya çalışmaktadırlar. Bu çalışmada, mukozal ödemde rol oynayan inflamatuar mediatörlerden olan indüklenebilir nitrik oksit sentetaz (iNOS), siklooksijenaz 2 (COX-2) ve vasküler endotelyal growth faktör (VEGF)'ün sinonazal polip dokusundaki ekspresyon seviyelerini araştırmak ve kortikosteroid tedavisinin bu mediatörlerin ekspresyon seviyelerine etkisini incelemek amaçlandı. Bu çalışmaya dahil edilen 22 nazal polipli hastadan, kortikosteroid tedavisi öncesi ve sonrası, lokal anestezi altında doku biyopsisi alındı. Alınan biyopsiler, immünohistokimyasal yöntemle boyanarak, aynı patolog tarafından, ödem, eosinofili, lenfoplazmositer infiltrasyon derecesi ve iNOS, COX-2 ve VEGF ekspresyon seviyesi açısından değerlendirildi. Ayrıca hastalar kortikosteroid tedavisi öncesinde ve sonrasında polip evresi ve subjektif semptomlar yönünden değerlendirildi. Sonuç olarak; kortikosteroid tedavisinin, nazal polip dokusunda immünohistokimyasal olarak, iNOS ve VEGF ekspresyon seviyesinde istatistiksel olarak anlamlı azalmaya neden olduğu saptandı (p=0.023, p=0.037). COX-2 ekspresyon seviyesinde ise istatistiksel olarak anlamlı bir değişiklik saptanmadı (p=0.307). Ayrıca, dokudaki ödem, eosinofili ve lenfoplazmositer infiltrasyon derecesinde tedavi öncesinde ve sonrasında herhangi bir farklılık saptanmadı (p>0.05). Polip evrelemesinde ve subjektif semptomlarda ( burun tıkanıklığı, rinore, koku duyusu kaybı, geniz akıntısı, alerji, baş ve yüz ağrısı ) kortikosteroid tedavisinden sonra istatistiksel olarak anlamlı gerileme bulundu. Bu çalışmanın sinonazal poliposis etyopatogenezinde rol oynayan mediatörleri inceleyen ve kortikosteroid tedavisinin nazal polipozisdeki etki mekanizmasını araştıran sınırlı sayıdaki çalışmaya katkı sağlayabileceği fakat daha kesin sonuçlara ulaşabilmek için olgu sayısının arttırıldığı ileri çalışmalara ihtiyacın olduğu belirlendi.Bu çalışma Ç.Ü.
