Genetic risk factors in oligodendrogliomas and oligoastrocytomas and the effects of these factors on prognosis.

(bzs. rnk.), tablo ; 29 cm.Purpose: In this study, we aimed to investigate whether there is relationship between 1p and 19q deletions and prognosis in 15 cases with oligodendrogliomas which we did surgical treatment in our clinic between 2001 and 2010. Material and Method: 15 cases with oligodendroglial tumours that were operated in our clinic between 2001 and 2010 were determined by receiving faculty ethical committee and patient approvals. 1p and 19q deletions were investigated with fluoresence in situ hybridization method in the formaline - fixed paraffin sections. According to the deletion results, we devided these cases into 3 groups consisting of 5 as having both deletions(Group 1), having only 1p deletion (Group 2) and having neither of deletions (Group 3). And these groups are compared in terms of median ages during the diagnosis, median follow-up time and median progression free follow-up time and it was investigated whether there is relationship between these deletions and prognosis. Results: 1 of 15 cases whose pathology samples were studied with fluoresence in situ hybridization method was anaplastic oligoastrocytoma, 14 were oligodendroglioma grade 2 tumours. It was determined that in 5 of these 15 cases, both of 1p and 19q deletions exist . While only 1p deletion was determined in 5 cases, neither of these deletions was determined in 5 cases left. 2 of oligodendroglioma grade 2 tumours showed malign tranformation to anaplastic oligoastrocytoma The median ages of cases during the diagnosis were 37,4 in Group 1, 41,8 in Group 2 and 34,2 in Group 3 and statistically no meaningful differences were found in terms of this parameter (p>0,05). For those having median follow-up time co-deletion was (Group 1) 28,6 months, for Group 2, 44,4 months and for Group 3, 63 months. Statistically no meaningful differences were found in terms of average follow-up time, either. Death resulting from tumour was seen in only one case (%6,7) and this case was in the group having neither deletions (Group 3). Therefore, no comparison could be done in terms of life time. While the rate of relapse for the group having co-deletion was % 0, it was determined as % 20 in the group having only 1p deletion and % 40 in the group having no deletion. Statistically, no meaningful differences were found when cases were evaluated in terms of rate of relapses, either (p>0,05). Median follow-up time was 38,2 months for Group 1, 48,6 months for Group 2 and 52,8 months for Group 3. median progression-free follow-up time was 38,2 months in the group having co-deletion since there was no fact showing progression, 23, 6 months in Group 2 and 40, 8 months in Group 3. Statistically, no meaningful differences were found among the groups for each of these parameters (p>0,05). Conclusion: In this study, due to the number of our cases was limited with 15, enough datas couldn?t be collected to get meaningful results statistically and statistical analiysises could be no more than definitions. In the light of these evaluations that are now routinely performed for these tumours all over the world, we carried out this study with the aim of being accepted as preliminary study, being able to form basis for future studies about molecular markers in oligodendroglial tumours and to light the way for some problems.Amaç: Biz bu çalışmamızda, kliniğimizde 2001 ile 2010 yılları arasında cerrahi tedavi uyguladığımız oligodendroglial tümörlü 15 olguda, 1p ve 19q delesyonları ile prognoz arasında herhangi bir ilişki olup olmadığını araştırmayı amaçladık. Materyal ve Metod: Fakülte etik kurulu onayı ve hasta onayları alınarak, kliniğimizde 2001 ile 2010 yılları arasında oligodendroglial tümör tanısı almış 15 olgu belirlendi. Bu olguların formalinle fikse edilmiş parafin kesitlerinde Fluoresence in situ hybridization yöntemiyle 1p ve 19q delesyonları araştırıldı. Delesyon sonuçlarına göre bu olguları her iki delesyona sahip olanlar (Grup 1), sadece 1p delesyonuna sahip olanlar (Grup 2) ve delesyonlardan hiçbirine sahip olmayanlar olarak (Grup 3) 5?er olgudan oluşan 3 gruba ayırdık. Ve bu gruplar, tanı anındaki ortalama yaşları, ortalama izlem süreleri ve ortalama progresyonsuz izlem süreleri baz alınarak karşılaştırıldı ve bu delesyonlarla prognoz arasındaki ilişki olup olmadığı araştırıldı. Bulgular: Fluoresence in situ hybridization yöntemiyle patoloji örnekleri incelenen 15 olgudan 1 tanesi anaplastik oligoastrositom, 14 tanesi ise grade 2 oligodendroglial tümörlerdi. Bunların arasından 2 tanesi anaplastik oligoastrositoma malign transformasyon gösterdi. Bu 15 olgunun 5 tanesinde 1p ve 19q delesyonların her ikisinin mevcut olduğu tespit edildi. 5 olguda sadece 1p delesyonu saptanırken, geriye kalan 5 olguda bu iki delesyondan hiçbirisine rastlanmadı. Oligodendroglial grade 2 tümörlerden iki tanesi anaplastik oligoastrositoma malign transformasyon gösterdi. Grup 1? deki olguların tanı anındaki ortalama yaşları 37,4, Grup 2?dekilerin ki 41,8, Grup 3?dekilerin ise 34,2 idi ve bu parametre açısından gruplar arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05). Ortalama izlem süresi kodelesyona sahip olanlar (Grup 1) için 28,6 ay, Grup 2 için 44,4 ay, Grup 3 için ise 62 aydı. Ortalama izlem süresi açısından da istatistiksel olarak anlamlı farklılık saptanmadı. Tümöre bağlı ölüm sadece bir olguda görüldü ( %6,7) ve bu olgu her iki delesyona da sahip olmayan (Grup 3) olgu grubundaydı. Bu nedenle bu 3 grup arasında yaşam süreleri temel alınarak bir karşılaştırma yapılamadı. Nüks oranı kodelesyona sahip grup için % 0 iken, sadece 1p delesyonuna sahip grupta % 20, delesyonu olmayan grupta ise % 40 olarak belirlendi. Nüks oranları açısından olgular grup bazında değerlendirildiğinde de istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05). Ortalama takip süresi Grup 1 için 38,2 ay, Grup 2 için 48,6 ay, Grup3 için ise 52,8 aydı. Ortalama progresyonsuz takip süreleri ise kodelesyona sahip grupta progresyon gösteren olgu olmadığından yine 38,2 ay, Grup 2?de 23,6 ay, Grup 3?te ise 40,8 ay idi. Bu parametrelerin her birisi için gruplar arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05). Sonuçlar: Çalışmamızda olgu sayımızın 15 ile sınırlı kalması nedeniyle istatistiksel olarak anlamlı sonuç almaya yetebilecek veriler toplanamadı ve yapılan istatistiksel analizler tanımlayıcı olmaktan öteye gidemedi. Biz bu çalışmamızı, tüm dünyada artık oligodendroglial tümörlerde rutin olarak yapılan bu değerlendirmelerin ışığında öncül bir çalışma olarak kabül edilmesi, oligodendroglial tümörlerdeki moleküler markerlarla ilgili ilerleyen yıllarda yapılacak çalışmalar için temel oluşturabilmesi ve bazı sorunlara ışık tutabilmesi amacıyla tamamladık.Bu çalışma Ç.Ü.
