Estudio sobre la viabilidad y eficacia del protocolo unificado para el tratamiento transdiagnóstico de los trastornos emocionales en un contexto de salud mental público.

Emotional disorders (anxiety, depression and related disorders) are the most prevalent psychological disorders according to recent epidemiological studies. Due to their high prevalence in the population, finding a cost-effective treatment for these disorders has become a key objective for clinical psychology, especially in public contexts where limited resources and face long waiting lists are becoming an alarming problem. A transdiagnostic approach, such as the Unified Protocol (UP), could serve this purpose, since it can treat a variety of disorders simultaneously and can be easily applied in a group format. The aims of this work are to explore the feasibility of applying the UP for the treatment of emotional disorders in a group format in a public health context, as well as to compare its efficacy with traditional individual cognitive-behavioral therapy. Method. Participants were 29 patients with emotional disorders who were randomized to the Usual Treatment (TAU; individual cognitive-behavioral therapy, CBT) (n = 13) or to the UP condition in group format (n = 16). At the beginning and the end of the study, participants responded to a diagnostic clinical interview based on the DSM-5 criteria, as well as to self-reports on depression, anxiety and quality of life. Results. Feasibility results indicated that patient acceptance (frequency of session attendance and drop-out rates) was satisfactory. Regarding the treatment effectiveness, multivariate analyses of variance (MANOVA) showed significant changes in both groups (TAU and UP) in all study variables, namely depression (F = 26.65; p <0.001, ηp² = 0.61), anxiety (F = 15.64; p = 0.001, ηp² = 0.48) and quality of life (F = 5.38; p = 0.033, ηp² = 0.24). When analyzing whether there were differences in the scores as a function of treatment group, we only found significant differences in depression, where there was a greater improvement in the UP group (F = 5.42; p = 0.033, ηp² = 0.24). Discussion. Results suggest that both the UP and TAU are similarly effective treatments to reduce the symptoms of patients with emotional disorders after 3 months of intervention. On the one hand, patients in the TAU condition received less sessions, but their effectiveness results were mostly comparable to the UP condition, which would make the former a more efficient option in this case. On the other hand, the UP condition presented a better adherence, a saving of time when training therapists in the use of a single protocol instead of multiple treatments and the use of a group format that allowed to attend several patients at the same time being able to offer a greater number of sessions in the same space of time. Also, improvements in depression were slightly larger. Thus, the use of the UP in group format can be an appropriate method for the treatment of emotional disorders in public mental health contexts considering the feasibility and an efficacy data, together with practical and clinical advantages over individual traditional CBT.Introducción. Los trastornos emocionales (ansiedad, depresión y trastornos relacionados) son los trastornos psicológicos más prevalentes según estudios epidemiológicos recientes. Debido a su alta prevalencia en la población, encontrar un tratamiento coste-efectivo para estos trastornos se ha convertido en un objetivo clave para la psicología clínica, sobre todo en contextos públicos donde cuentan con recursos limitados y se enfrentan a largas listas de espera. Un abordaje transdiagnóstico, como por ejemplo el Protocolo Unificado (PU), podría servir a este propósito, ya que puede aplicarse a una variedad de trastornos simultáneamente y puede ser fácilmente aplicado en formato grupal. Los objetivos de este trabajo son explorar la viabilidad de aplicar el PU para el tratamiento de trastornos emocionales en formato grupal en un contexto público de salud, así como comparar su eficacia con el tratamiento cognitivo-conductual individual tradicional. Método. Los participantes fueron 29 pacientes con trastornos emocionales que se asignaron al azar a la condición Tratamiento Habitual (TH; terapia cognitivoconductual individual) (n = 13) o a la condición de PU en formato grupal (n = 16). Al inicio y al final del estudio los participantes respondieron a una entrevista clínica diagnóstica basada en los criterios del manual DSM-5 y una entrevista semiestructurada de cribaje, además de a autoinformes sobre depresión, ansiedad y calidad de vida. Resultados. Los resultados sobre viabilidad indican que la aceptación por parte de los pacientes (frecuencia de asistencia a sesiones y tasas de abandono) ha sido satisfactoria. En cuanto a la efectividad de los tratamientos, los análisis multivariados de la varianza (MANOVA) mostraron cambios significativos en ambos grupos (TH y PU) en todas las variables de estudio, es decir depresión (F = 26,65; p < 0,001, ηp² = 0,61), ansiedad (F = 15,64; p = 0,001, ηp² = 0,48) y calidad de vida (F = 5,38; p = 0,033, ηp² = 0,24). Al analizar si existían diferencias en las puntuaciones por grupo de tratamiento, únicamente encontramos diferencias significativas en depresión, donde se evidenció una mayor mejoría en el grupo PU (F = 5,42; p = 0,033, ηp² = 0,24). Discusión. Los resultados sugieren que tanto el PU como el TH son tratamientos eficaces para reducir los síntomas de los trastornos emocionales tras 3 meses de intervención. Por una parte, los pacientes de la condición TH recibieron un menor número de sesiones obteniendo resultados comparables en eficacia a la condición PU y por ello sería una opción más eficiente en este caso. Por otra parte, la condición de PU presentó una mejor adherencia, un ahorro de tiempo al entrenar a los terapeutas en el uso de un único protocolo en lugar de múltiples y el uso de un formato grupal que permitió atender a varios pacientes a la vez pudiendo ofrecer un mayor número de sesiones en el mismo espacio de tiempo. Así pues, la utilización del PU en formato grupal puede ser un método adecuado para el tratamiento de los trastornos emocionales en contextos de salud mental pública al presentar datos adecuados de viabilidad y una eficacia comparable al TH además de presentar ciertas ventajas prácticas y clínicas respecto al TH individual
