Mortalidad temprana en pacientes con infarto miocárdico agudo tratados con estreptoquinasa recombinante

Although thrombolysis improves the prognosis of the patients with acute myocardial infarction, an increase of mortality on the first day has been described. This paper, which is based on a recently finished study where 2 915 patients with this affection were treated with Recombinant Streptokinase, is aimed at knowing retrospectively the distribution of deaths in the first 7 days after thrombolysis, the relationship between letality on the first day and the causes of death during the following days and their distribution in time. The number of deaths was higher in the first 2 days (27,8 and 21,5%), whereas letality on the fisrt day was significantly higher in patients over 60 and women. The fail of the pump was the most frequent cause of death followed by heart rupture and arrhytmias, which ocurred mainly in the first 3 days after infarction.Aunque la trombólisis mejora el pronóstico de los pacientes con infarto miocárdico agudo se ha descrito incremento de la mortalidad el primer día. En un estudio terminado recientemente en el que se trató con Estreptoquinasa Recombinante a 2 915 pacientes con esa afección, se realizó el presente trabajo para conocer retrospectivamente, la distribución de las muertes en los primeros 7 días después de trombólisis, la relación entre la letalidad del primer día y las causas de muerte en los días posteriores y su distribución en el tiempo. El número de fallecidos fue mucho mayor en los 2 primeros días (27,8 y 21,5 %) y la letalidad del primer día fue significativamente más alta en los mayores de 60 años y en las mujeres. El fallo de bomba fue la causa de muerte más frecuente seguido por la rotura cardíaca y las arritmias. Estas ocurrieron fundamentalmente en los 3 primeros días después del infarto
