Effects of choline treatment on acute phase proteins and cardiac functions in dogs with endotoxemia

Bu çalışmada; 1) endotoksine yanıt olarak akut faz proteinlerinin (AFP) ve high mobility group box-1 (HMGB-1) proteinde oluşan değişimlerin diyagnostik değerlerinin ortaya konması; 2) miyokardiyal hasarın kardiyak biyobelirteçler ve sistolik zaman aralıkları (STI) ile birlikte miyokardiyal performans indeksinin (Tei index) Doppler ekokardiografik ölçümlerle belirlenebilmesi ve 3) intravenöz kolin ve CDP-kolin tedavisi ile endotoksemili köpeklerde kardiyak anormalitelerin yanısıra AFP'leri ve HMGB-1 yanıtlarındaki değişimlerin belirlenmesi amaçlanmıştır.Çalışmada herbirinde 6 köpeğin bulunduğu 6 tedavi grubu kullanıldı. Çalışmanın ilk bölümünde, intravenöz salin (grup I), kolin klorid (grup II) ve CDP-kolin (grup III) verilen köpekler, klinik ve laboratuar parametreleri yönünden 48 saat süresince monitorize edildi. Çalışmanın ikinci bölümünde köpeklere, grup I, II, ve III'e ek olarak, endotoksin (E coli, lipopolisakkarid [LPS], iv) verilmiş, LPS + Salin (grup IV); LPS + Kolin klorid (grup V) ve LPS + CDP-kolin (grup VI) oluşturulmuştur. Kan örnekleri endotoksin enjeksiyonundan önce (0.saat) ve sonra 0.5, 1, 4, 24 ve 48. saatlerde toplanmıştır.Yalnız LPS alan köpeklerde 0,5-48. saat diliminde sistemik inflamatuar yanıt sendromu (SIRS) tespit edilmiştir. Kolin tedavisi LPS-ilişkili SIRS bulgularını engellemiş veya minimalize etmiş, AFP'leri düzeylerinde (C reaktif protein haricinde) ve HMGB-1 düzeylerinde koruma sağlamıştır. Kardiyak disfonksiyon pre-ejeksiyon (PEP) ve sol ventriküler ejeksiyon sürelerinde (LVET) kısalma, kardiyak output (CO) ve stroke volüm (SV) değerlerinde azalma ve Tei indekste artma ile karakterize edilmiştir. Miyokardiyal hasar serum LDH, CK ve AST'de artma ile Mg seviyelerinde azalma ile ortaya konmuştur. Kolin tedavisi (kolin veya CDP-kolin olarak) STI, kardiyak biyobelirteçler ve Tei indeksteki artış veya azalmalara karşı koruma sağlamıştır.Köpeklerdeki bulgularımız; 1) kolin tedavisinin akut faz yanıtlarını ve HMGB-1 protein davranışını düzenleyerek SIRS gelişimini engelleyebileceğini (anti-inflamatuar etki); 2) HMGB-1 proteininin endotoksemiden şüphelenilen köpeklerde erken yangısal belirteç olarak kullanılabileceğini; 3) Tei indeks'in sepsis ilişkili miyokardiyal performans değerlendirilmesinde rutin olarak kullanılabileceğini ve 4) köpeklerde intravenöz kolin tedavisinin kardiyak yetmezlik ve LPS-ilişkili miyokardiyal hasarın minimalize edilmesinde etkili olabileceğini göstermektedir.In this study, we aimed to investigate; 1) diagnostic values of the changes in serum acute phase proteins (APPs) and high mobility group box-1 (HMGB-1) protein levels in response to endotoxin; 2) presence of myocardial injury and cardiac function impairment based on the serum cardiac biomarkers and Doppler derived echocardiographic measurements of systolic time intervals (STI) and myocardial performance index (Tei index); and 3) potential of intravenous choline treatment to reverse APP and HMGB-1 responses as well as cardiac abnormalities in dogs with endotoxemia.Six treatment groups (with 6 dogs in each group) were used. In the first series of the experiment, intravenous normal saline (group I), choline chloride (group II) or CDP-choline (group III) was given to dogs which were monitored for clinical and laboratory parameters during a 48 h period. In the second series of the experiment, in addition to treatment plans in group I, II, and III, endotoxin (E coli, lipopolysaccharide [LPS], iv) was given to dogs: LPS + Saline (group IV); Choline chloride + LPS (group V) and CDP-choline + LPS (group VI). Blood samples were collected, before and 0.5, 1, 4, 24, and 48 hrs after endotoxin injection.Systemic inflammatory response syndrome (SIRS) was characterized at 0,5-48 hrs in dogs receiving LPS alone. Choline treatment either blocked or minimized LPS-associated SIRS and exhibited protection on levels of APPs (except for C reactive protein) and HMGB-1. Cardiac dysfunction was characterized by shortened preejection period (PEP) and left ventricular ejection time (LVET), and decreased cardiac output (CO) and stroke volume (SV) in combination with tendency of Tei index value to increase. Myocardial injury was characterized by increased serum LDH, CK and AST and decreased Mg level. Choline treatment (either as choline or CDP-choline) also protected from increases or decreases in STI, cardiac biomerkers and Tei index after LPS.Our findings suggest in dogs that 1) choline treatment may prevent the development of SIRS by regulating acute phase response and HMGB-1 protein behaviors associated with LPS (anti-inflammatory effect); 2) HMGB-1 protein may be used an early inflammatory marker in dogs suspected of endotoxemia; 3) Tei index can be used routinely for the evaluation of sepsis-related myocardial performance and 4) intravenous choline treatment can be effective in the treatment of cardiac dysfunction and LPS-related myocardial injury in dogs.TÜBİTA
