Orbital involvement in sarcoidosis usually is primary; however, rare patients have been reported in whom sarcoidosis primarily affected the paranasal sinuses and secondarily invaded the orbit. Most patients in whom sarcoidosis affects one or both orbits have known sarcoidosis at the time of initial symptoms. Other patients may not be known to have sarcoidosis at the time of orbital manifestations but have other evidence of sarcoidosis, particularly bilateral hilar adenopathy, when a thorough evaluation is performed
