Although uncommon, metastasis to the optic nerve have been documented either in isolation or as an extension from central nervous system, orbital, retinal, or choroidal metastatic foci. We report a case of bilateral anterior optic neuropathy in a patient with metastatic breast cancer treated by optic nerve sheath fenestration with improvement in visual acuity and resolution of the disc edema
