A 27 year old college educated young woman presented with a progressive history of ataxia and involuntary movement that had been noted at one year of age. Prominent conjunctival vasculature was evident and she was confined to a wheelchair. Recently, reading had deteriorated and she could not cross-stitch. See-saw nystagmus was observed with a superimposed alternating nystagmus
