Retinitis pigmentosa induces oxidative damage and inflammation in humans: looking for peripheral biomarkers

[EN] Retinitis Pigmentosa (RP) is a common form of hereditary retinal degeneration characterized by the loss of retinal photoreceptor cells (rods and cones) and constitutes the largest single cause of blindness in the developed world. It can be associated with other diseases giving rise to a serial of syndromes such as Usher syndrome. It is difficult to understand the pathogenetic mechanisms responsible for apoptosis of photoreceptor cells due to its high genetic heterogeneity. Genetic mutations are responsible for the apoptosis of rods. However, the death of cones seems to be due to metabolic changes caused by the degeneration of rods (oxidative stress, cytokines, etc.). The degeneration of rods generates hyperoxia that causes oxidative and nitrosative stress. Furthermore, rods and other surrounding cells alter secretion of growth factors, pro/anti-angiogenic factors, cytokines, etc. affecting the survival of cones. It is important to study the environment surrounding these cones since its death causes loss of central vision in the RP. The main objective of this project is to evaluate the retinal environment in patients with non-syndromic RP and in patients with Usher syndrome through determination of soluble factors in aqueous humour which may affect the photoreceptor condition. We will evaluate the antioxidant defense system and the presence of inflammatory mediators in this biological fluid. This will allow us a better understanding of the microenvironment surrounding photoreceptors to develop successful treatments improving its survival and thus the vision of RP patients. It hopes to establish a relation between severity of RP and the ocular levels of these factors. Finally we expect to find peripheral blood biomarkers that can reflect the status of the retina in RP patients[ES] La retinosis pigmentaria (RP) es una forma común de degeneración retiniana hereditaria caracterizada por la pérdida de los fotorreceptores de la retina (conos y bastones) y constituye la principal causa genética de ceguera en los países desarrollados. Puede estar asociada a otras patologías dando lugar a diversos síndromes como el síndrome de Usher. Debido a su elevada heterogeneidad genética, es difícil comprender los mecanismos moleculares que causan la muerte de los fotorreceptores. Las mutaciones genéticas son las responsables de la apoptosis de los bastones. Sin embargo, la muerte de los conos parece debida a los cambios metabólicos que provoca la degeneración de los bastones (estrés oxidativo, citoquinas, etc.). La degeneración de los bastones genera hiperoxia causante de estrés oxidativo y nitrosativo. Además, los bastones y otras células del entorno alteran la secreción de factores de crecimiento, factores pro/anti-angiogénicos, citoquinas, etc. afectando a la supervivencia de los conos. Es importante estudiar el entorno que rodea a los conos ya que su muerte provoca la pérdida de la visión central en RP. El objetivo principal de este proyecto es evaluar el microambiente retiniano en pacientes con RP no sindrómica y en pacientes con síndrome de Usher a través de la determinación en humor acuoso de factores solubles que pueden afectar a la supervivencia de los fotorreceptores. Se evaluará el estado antioxidante y la presencia de mediadores inflamatorios en este fluido biológico. Esto nos permitirá conocer mejor el microambiente que rodea a los fotorreceptores con el fin de diseñar tratamientos que mejoren su supervivencia y con ello la visión de los pacientes con RP. Se espera poder establecer alguna relación entre la severidad de la RP y los niveles oculares de alguno de estos factores.
