La contraception du post-partum : les stratégies d'information, le point de vue des femmes

Objective : In France, 5,5% of the voluntary terminations of pregnancy take place in the six months that follow the childbirth. This renewed report of the persistence of unplanned pregnancy in the post-partum raises the question of the strategies which are being organised to inform women about this contraception. This information is essential to allow the young mothers to choose the contraceptive method the most adapted to their new situation. Indeed, as reminds regularly E Aubeny, "the best contraception is not the most effective in theory, but the one that the woman really chose". The objective of this study is to estimate this information through the women's point of view. Methods : A hundred and four young mothers were questioned from September until December 2013. During their session of perineal rehabilitation by a liberal midwife, they filled a questionnaire concerning the information delivered to them on the contraception of the post-partum. Results and conclusion : In our study, almost a third of women who gave birth (28,7%) are not satisfied by the information they received. 56,7% of them found the moment in which this information was explained to them inappropriate. As for the women who were satisfied, they found the moment appropriate to more than 95%. None of the women of the study knew all the possible methods after childbirth. And among the women who had a prescription at the release from the maternity, 30,8% did not follow it and 69,4% changed it during the postnatal visit. Information about the contraception of the post-partum, although paramount, is not optimal. It seems thus essential to find how and by which means we can have a better approach to the question of contraception with pregnant or just-giving birth women. For that purpose, it is necessary to fit out times at convenient moments and thus to allow the woman to develop her choice in the purpose of planning of a future wished pregnancy.Objectifs : En France, 5,5% des interruptions volontaires de grossesse ont lieu dans les six mois qui suivent l'accouchement. Ce constat renouvelé de la persistance de grossesse non prévues dans le post-partum fait poser la question des stratégies mises en place pour informer la femme sur cette contraception. Cette information est indispensable pour permettre aux jeunes mères de choisir la méthode contraceptive la plus adaptée à leur nouvelle situation. En effet, comme le rappelle régulièrement E. Aubeny « la meilleure contraception n'est pas la plus efficace théoriquement, mais celle que la femme a vraiment choisi ». L'objectif de cette étude est d'évaluer cette information par l'intermédiaire du point de vue des femmes. Matériel et méthodes : Cent quatre jeunes mères ont été interrogées de septembre à décembre 2013. Durant leur séance de rééducation périnéale par la sage-femme libérale, elles ont rempli un questionnaire concernant l'information qui leur a été délivrée sur la contraception du post-partum. Résultats : Dans notre étude, presque un tiers des accouchées (28,7%) n'est pas satisfaite de l'information reçue et 56,7% d'entre elles ont trouvé le moment où cette information leur a été expliquée inadapté. Les femmes qui ont été satisfaites ont quant à elles trouvé le moment adapté à plus de 95%. Aucune des femmes de l'étude ne connaissait toutes les méthodes possibles après un accouchement. Et parmi les femmes ayant eu une ordonnance à la sortie de la maternité, 30,8% ne l'ont pas suivie et 69,4% en ont changé lors de la visite post-natale. L'information sur la contraception du post-partum bien que nécessaire n'est pas optimale. Il apparaît donc primordial de trouver comment et par quels moyens mieux aborder la question de la contraception auprès des femmes enceintes ou venant d'accoucher. Pour cela, il est nécessaire d'aménager des temps à des moments opportuns et ainsi permettre à la femme d'élaborer son choix dans un but de planification d'une future grossesse désirée
