Meningitis tuberculosa: a propósito de dos casos

Background: Nine million cases of tuberculosis (TB) occur worldwide annually, about one million of those cases (11%) occur in children under 15 years of age. In our environment represent 1,5% TB meningitis. Description of the cases: We report two cases of TB meningitis admitted to a tertiary hospital. In both, diagnostic difficulties were encountered. They were treated with tuberculostatic for 12 months and with glucocorticoid for 4 weeks. One case developed a ventricle-peritoneal shunt. The evolution was very satisfactory in both children. Conclusions: The diagnosis of TB meningitis in children relies from a careful history, clinical examination, tuberculin skin test, chest radiograph, sputum o gastric aspiration and lumbar puncture. The treatment regimen comprising isoniazid, rifampicin, pyrazinamide andethambutol or streptomycin for the first 2 months, followed by isoniazid and rifampicin for 10 months. A glucocorticoid was added during the first month of treatment.Antecedentes: De los 9 millones de tuberculosis (TB) que ocurren anualmente en el mundo, alrededor de un millón suceden en niños menores de 15 años. En nuestro medio el 1,5% son meningitis TB. Descripción de los casos: Se exponen dos casos de meningitis TB ingresadas en un hospital terciario. En ambos se encontraron dificultades para realizar el diagnóstico. Fueron tratados con tuberculostáticos durante 12 meses y glucocorticoides durante 4 semanas. En un caso hubo que poner una derivación ventrículo peritoneal. La evolución fue muy satisfactoria en los dos pacientes. Conclusiones: El diagnóstico de meningitis TB en niños requiere historia clínica cuidadosa, examen físico, test de tuberculina, radiografía de tórax, examen de esputo o de jugo gástrico y punción lumbar. El tratamiento incluyó isoniazida, rifampicina, pirazinamida y ethambutol o estreptomicina durante 2 meses, seguido de isoniazida y rifampicina 10 meses más. Un glucocorticoide fue añadido durante el primer mes del tratamiento
