Okołodobowe stężenia DHEA a stan kośćca u otyłych kobiet po menopauzie

INTRODUCTION. It has been suggested that increased DHEA secretion in postmenopausal obese women might affect bone tissue; the association might be significantly influenced by OPG and its ligand, RANKL. The aim of the study was to determine whether postmenopausal obese women showed any relationship between circadian concentrations of DHEA and BMD and bone metabolism, and to establish whether OPG and RANKL might play a role in the relationship. MATERIAL AND METHODS. Thirty-five postmenopausal women (20 obese and 15 healthy individuals) underwent BMD of the lumbar spine (L2–L4) by dual-energy X-ray absorptiometry (DXA); serum DHEA, bone markers (OC and CTx), OPG and its soluble ligand sRANKL levels were determined by ELISA in serum samples collected every three hours. RESULTS. A significant increase in body mass, BMI and WHR indices and BMD in obese women was accompanied by a significant increase of mean circadian concentration of DHEA, and a decrease of mean circadian concentrations of OC, CTx, OPG, and sRANKL. The amplitudes of OC, CTx, OPG and sRANKL rhythms were also decreased, and a shift was observed in sRANKL rhythm acrophase. Positive correlations were revealed between BMD and DHEA and OPG/sRANKL ratio. A negative correlation was found between BMD and sRANKL. Circadian concentrations of DHEA were negatively correlated with OC, CTx and sRANKL and positively with OPG and OPG/sRANKL ratio. A negative correlation was seen between circadian concentrations of sRANKL and OC and OPG. CONCLUSIONS. Postmenopausal obesity results in elevation of circadian DHEA secretion associated with increased BMD, decreased bone metabolism, and suppression of circadian OPG and sRANKL oscillations. Increased DHEA secretion in postmenopausal obese women may beneficially affect bone tissue most probably via a shift in the OPG/RANKL ratio that tilts the balance toward a functional excess of OPG.WSTĘP. Istnieją sugestie, że zwiększone wytwarzanie dehydroepiandrosteronu (DHEA) u otyłych kobiet po menopauzie może wpływać na tkankę kostną, a istotną rolę w mechanizmie tych powiązań mogą odgrywać osteoprotegeryna (OPG) i jej ligand — RANKL. Celem badań było wykazanie, czy u otyłych kobiet po menopauzie istnieje związek między okołodobowymi stężeniami DHEA a gęstością mineralną kości (BMD) i metabolizmem kostnym oraz ustalenie, czy OPG i RANKL mogą mieć znaczenie w mechanizmie tych zależności. MATERIAŁ I METODY. U 35 kobiet po menopauzie (20 otyłych i 15 zdrowych) wykonano badanie BMD metodą dwuwiązkowej absorpcjometrii rentgenowskiej (DXA) obejmujące część lędźwiową kręgosłupa L2–L4 oraz, metodą ELISA, oznaczono stężenia DHEA, markerów kostnych (OC i CTx), OPG i jej rozpuszczalnego liganda sRANKL w surowicy krwi pobieranej w odstępach 3-godzinnych w ciągu doby. WYNIKI. Istotnemu zwiększeniu masy ciała, wskaźnika masy ciała (BMI), wskaźnika talia–biodra (WHR) i BMD u otyłych kobiet towarzyszyło istotne zwiększenie średniodobowego stężenia i amplitudy okołodobowych oscylacji DHEA, obniżenie średniodobowych stężeń OC, CTx, OPG, sRANKL i amplitudy rytmów OC, CTx, OPG, sRANKL oraz przesunięcie akrofazy rytmu sRANKL. Wykazano dodatnią korelację między BMD a DHEA i wskaźnikiem OPG/sRANKL oraz ujemną z sRANKL. Okołodobowe stężenia DHEA korelowały ujemnie z OC, CTx i sRANKL, a dodatnio — z OPG i wskaźnikiem OPG/sRANKL. Stwierdzono ujemną korelację między okołodobowymi stężeniami sRANKL a OC i OPG. WNIOSKI. Otyłość u kobiet po menopauzie prowadzi do wzrostu okołodobowego wydzielania DHEA, któremu towarzyszy zwiększenie BMD, zmniejszenie metabolizmu kostnego oraz supresja okołodobowych oscylacji OPG i sRANKL. Zwiększone wydzielanie DHEA u otyłych kobiet po menopauzie może korzystnie wpływać na tkankę kostną, najprawdopodobniej poprzez przesunięcie relacji OPG do sRANKL na rzecz OPG
