Essential thrombocythemia is a chronic myeloproliferative disorder which characterizes of increasing platelets, recurrent veno- and arteria thrombosis and occasionally bleeding. Cerebral ischaemic stroke can occur during the disease. The autors describe a case of 67-year old women with essential thrombocythemia with ischaemic stroke treated recombinant tissue plasminogen activator (rt-PA) with success. The hemiparesis resolved, but hemorrhagic diathesis as well as thrombophlebitis in lower limb occurred after treatment. Application of heparin and aspirin with hydroxacarbamid chronically proved to be effective.
