Choroby alergiczne wieku podeszłego. Część I. Alergiczny nieżyt nosa i zatok, astma oskrzelowa

During the last century the average length of the human life has become nearly thirty years longer, so there is more older people now than in the former centuries. Thus there appeared a problem of allergic diseases of the elderly. The allergic diseases may reveal in the youthful period and have many years’ course, or they may begin in the older age. In some cases the symptoms of the allergic diseases which began in the childhood may have the gentler course in the middle age and again grow worse in the elderly. In this article we have overviewed the allergic diseases of the respiratory system such as allergic rhinitis and the bronchial asthma. About 6% of the world population over 70 years old suffer from allergic rhinitis and the percentage of the patients with bronchial asthma is estimated on the level of 6.5-17%. The diagnostic and therapeutic difficulties of allergic rhinitis and bronchial asthma result from the altered reactivity of the immunological system, the anatomical and functional changes of the respiratory tract in the process of aging and also coexistence of different chronic diseases with simultaneous use of many medicines. In the therapeutic process we should pay attention to the patient’s quality of life and education concerning the proper technique of medication.Długość życia ludzkiego zwiększyła się w ostatnim wieku o blisko 30 lat, co spowodowało wzrost liczby ludzi w podeszłym wieku. Z tego też względu pojawił się problem chorób alergicznych wieku podeszłego. Choroby alergiczne mogą się ujawniać w okresie młodzieńczym i mieć wieloletni przebieg lub ich początek może przypadać na wiek podeszły. W niektórych przypadkach objawy chorób alergicznych, które rozpoczęły się w dzieciństwie, mogą mieć łagodniejszy przebieg w wieku średnim, ponownie nasilając się w okresie starości. W niniejszej publikacji omówiono choroby alergiczne układu oddechowego - alergiczny nieżyt nosa i astmę oskrzelową wieku podeszłego. Na świecie 6% populacji po 70. roku życia cierpi z powodu alergicznego nieżytu nosa, a odsetek chorujących na astmę w tej populacji szacuje się na 6,5-17%. Trudności w diagnostyce i leczeniu alergicznego nieżytu nosa i astmy oskrzelowej wynikają ze zmienionej reaktywności układu immunologicznego, zmian anatomicznych i czynnościowych układu oddechowego zachodzących w procesie starzenia, a także współistnienia innych chorób przewlekłych z równoczesną polipragmazją. W leczeniu należy głównie zwrócić uwagę na poprawę jakości życia i edukację pacjenta w zakresie właściwej techniki przyjmowania leków
