Echokardiograficzna ocena funkcji serca u osób chorujących na cukrzycę typu 2

Background: According to the data obtained from the country-wide NATPOL III research, the prevalence of diabetes in Poland stands at 5,6% The objective of the work was to assess the heart function of patients with type 2 diabetes by means of echocardiography. Material and methods: 106 patients were examined: 44 people with type 2 diabetes (gr D) and 62 healthy subjects, constituting the control group (gr C). Using the criteria of diabetes control, the first group was divided into 2 subgroups: 17 patients with controlled diabetes (gr cD) and 27 patients with uncontrolled diabetes (gr uD). Next, 2 subgroups were formed according to the duration of the disease: 23 patients suffering from diabetes &#8805; 10 years (gr mD) and 21 people suffering from diabetes < 10 years (gr lD). The examinations of the patients included electrocardiography and assessments of myocardial size and contractibility and diastolic activity of the left ventricle. Results: The echocardiographic examination revealed statistically significant differences between the groups D and C, between the groups mD, lD, and C, and between the groups cD, uD, and C. Moreover, group D additionally exhibited positive linear dependences between the duration of the disease and the diastolic interventricular septum thickness and between the duration of the disease and the mass index of the left ventricle. Conclusions: Myocardial differences observed in the echocardiographic image of a diabetic compared to a healthy person suggest myocardial dysfunction caused by diabetes. The existing differences between the group with controlled diabetes and the group with uncontrolled diabetes indicate that development of organic complications in diabetic patients can only be slowed down with procedures to normalise carbohydrate and lipid metabolic parameters and the arterial blood pressure.Wstęp: Według ogólnopolskiego badania NATPOL III rozpowszechnienie cukrzycy w Polsce wynosi 5,6%. Celem pracy była ocena funkcji serca za pomocą badania echokardiograficznego u osób chorujących na cukrzycę typu 2. Materiał i metody: Badaniami objęto 106 osób: 44 osoby z cukrzycą typu 2 (gr. D) oraz 62 zdrowe osoby, stanowiące grupę kontrolną (gr. C). Pierwszą grupę podzielono, stosując kryteria wyrównania cukrzycy na 2 podgrupy: 17 chorych z wyrównaną cukrzycą (gr. cD) oraz 27 chorych z niewyrównaną cukrzycą (gr. uD). Na podstawie kryterium czasu trwania choroby wyodrębniono następne 2 podgrupy: 23 osoby chorujące na cukrzycę 10 lat i dłużej (gr. mD) i 21 osób chorujących na cukrzycę krócej niż 10 lat (gr. lD). Wśród osób objętych badaniem wykonano badania echokardiograficzne i oceniono wymiary serca, kurczliwość mięśnia sercowego oraz czynność rozkurczową lewej komory. Wyniki: Stwierdzono różnice istotne statystycznie w badaniu echokardiograficznym pomiędzy grupą D a C, pomiędzy grupami mD, lD i C oraz grupami cD, uD i C. W grupie D występowały ponadto dodatnie zależności liniowe między czasem trwania choroby i grubością przegrody międzykomorowej w rozkurczu oraz między czasem trwania choroby i wskaźnikiem masy lewej komory. Wnioski: Stwierdzone w obrazie echokardiograficznym odrębności serca osoby chorującej na cukrzycę typu 2 w porównaniu z osobą zdrową mogą świadczyć o upośledzeniu funkcji serca jako konsekwencji cukrzycy. Występujące różnice pomiędzy grupą z wyrównaną cukrzycą i grupą z niewyrównaną cukrzycą wskazują na konieczność dążenia do normalizacji parametrów gospodarki węglowodanowej i lipidowej oraz wartości ciśnienia tętniczego u chorych na cukrzycę, jako na jedyne postępowanie mogące spowolnić występujący wraz z czasem trwania choroby rozwój powikłań narządowych cukrzycy
