Modelagem matemática e gerenciamento ótimo da infecção HIV-1/AIDS

Mathematical modeling of HIV-1 infection has proven to be instrumental for the modern understanding basis of the AIDS pathogenesis, since it offers the unique means to adequately pose hypotheses concerning AIDS dynamics and treatment protocols. Focusing on the HIV-1 subtype-B epidemic, this article presents a comprehensive review and discusses the state-of-the-art in the area. A more in-depth view of the infection at the cellular and molecular biology levels is developed and mathematical modeling techniques concerned with the natural history of AIDS pathogenesis are proposed. Ultimately, a general framework for the real-world problem of optimizing the highly active antiretroviral therapy (HAART) benefits is discussed regarding important questions associated with the drug chemotherapy resistance, side-effects and costs.A Modelagem matemática da infecção pelo HIV-1 provou-se instrumental para a moderna compreensão sobre a dinâmica da AIDS, pois oferece formas únicas de postular e avaliar adequadamente hipóteses relacionadas à dinâmica da infecção e aos protocolos de tratamento. Focando a atenção sobre a infecção pelo HIV tipo 1 clado B, este artigo resume e discute o estado da arte no tema. Uma análise mais detalhada no nível das biologias celular e molecular da infecção é, então, desenvolvida e técnicas de modelagem matemática para representação da dinâmica natural da patogênese são propostas. Por ultimo, uma estrutura geral para o problema prático da otimização da eficiência da terapia antiretroviral altamente ativa é discutida considerando-se questões fundamentais como resistência à terapia, efeitos colaterais e custo
