Various reports are available in medical and dental literature on the incidence of supernumerary teeth of man. However supernumerary teeth in permanent dentition rarely occur symmetrically and bilaterally. A 38-year old female with symmetrical supernumerary premolars in mandibular is reported. The crowns are normal in shape and size and resemble normal premolars and have fully formed roots. In addition to the study on supernumerary premolars, radiological examination revealed the presence of first molars on both sides which have three roots in the lower jaw
