Clinical and surgical comorbidities in patients with psoriatic arthritis

The psoriatic disease concept includes psoriasis (PsO) and psoriatic arthritis (PsA). PsO is a chronic relapsing inflammatory skin disease characterized by the presence of scaling lesions, typically localized on the extensor surfaces and the trunk. PsA is a chronic inflammatory skeletal disease associated with PsO. The estimated prevalence of PsA around the globe varies from 0.02 to 1% whereas its prevalence in patients with psoriasis ranges between 6 and 48%. Over the last decade therapeutic options for both skin and joint disease have dramatically improved thereby allowing efficient control of the primary disease manifestations in a majority of patients. The recent therapeutic progress has triggered increasing interest in a holistic approach towards psoriatic disease, with specific attention towards detection and treatment of the comorbidities seen in these chronic disease patients that contribute directly or indirectly to the mortality and morbidity in these patients.Comorbidities with a direct impact on mortality or morbidity may include malignancy, metabolic and cardiovascular diseases as well as depression. PsA was too long considered a relatively benign disorder, but it has become clear that 40 to 60% of all the patients will develop erosive arthritis, loss of joint architecture and associated loss of function clearly positioning PsA as a major health concern. So the aim of this study is to determine the prevalence and incidence of cardiovascular and metabolic comorbidities and orthopedic interventions in the patients with PsA and also to look for the risk factors which contribute to the development of cardiovascular comorbidities in these patients. Morover the study aims to identify the metabolic signatures in patients with PsA and controls by magnetic resonance spectroscopy of PBMC (Peripheral Blood Mononuclear Cells) and their correlation with severity of disease.status: publishe
