Nonspecific bronchial hyperreactivity in patients with allergic rhinitis and its relationship with serum eosinophilic cationic protein

Allerjik rinit ile astma arasındaki bağlantı uzun zamandan beri bilinmektedir. Allerjik rinit astma gelişmeden önce var olabilir. Astma semptomlarına sahip olmayan allerjik rinitli hastalar bronş hiperreaktivitesine sahip olabilir. Bu çalışmanın amacı allerjik rinitli hastalarda bronş hiperreaktivitesi sıklığını ve bronş hiperreaktivitesi ile serum eosinofilik katyonik protein seviyeleri arasında ilişki olup olmadığını araştırmaktır. Bu çalışmaya 49 allerjik rinitli hasta dahil edildi. Metakolin ile bronş uyarı testi yapıldı. Serum eosinofilik katyonik protein seviyeleri fluoroimmunoassay kit ile ölçüldü. Hastaların 18'de (%36.7) bronş hiperreaktivitesi saptandı. Bronş hiperreaktivitesine sahip yılboyu allerjik riniti olan hastaların yaş ortalaması, mevsimsel allerjik riniti olan hastalara göre daha küçüktü. Bronş hiperreaktivitesi sıklığı hastalık yaşının artması ile arttı. Bronş hiperreaktivitesi saptanan yılboyu allerjik rinitli hastalarda, serum eosinofilik katyonik protein seviyeleri bronş hiperreaktivitesi olmayan hastalardan anlamlı olarak daha yüksekti. Ancak serum eosinofilik katyonik protein seviyeleri ile bronş hiperreaktivitesi arasında korelasyon saptanmadı. Sonuç olarak, allerjik rinitli hastaların önemli bir kısmında solunum yollarına ait semptomlar olmasa bile bronş hiperreaktivitesi vardır.A link between allergic rhinitis and asthma has been known for a long time. Allergic rhinitis may precede to asthma. Nonasthmatic patients with allergic rhinitis may have bronchial hyperreactivity. The aim of this study was to investigate both the prevalance of bronchial hyperreactivity in patients with allergic rhinitis without any asthmatic symptoms and a possible relationship between bronchial hyperreactivity and serum eosinophil cationic protein. Fourthynine patients with allergic rhinitis were included in this study. Bronchial challenges were performed with increasing concentrations of metacholine. The serum concentrations of eosinophil cationic protein were measured with a commercially available fluoroimmunoassay kit. Bronchial hyperreactivity was found in 18 (36.7%) patients. The mean age of the patients with perennial allergic rhinitis who have bronchial hyperreactivity was less than the mean age of the patients with seasonal allergic rhinitis. The prevalance of bronchial hyperreactivity increased as the disease progressed, in the perennial allergic rhinitis group. Serum eosinophil cationic protein levels of patients with bronchial hyperreactivity were higher than those of patients without bronchial hyperreactivity. No correlation was found between serum eosinophil cationic protein levels and bronchial hyperreactivity. In conclusion, it seems like an important percent of allergic rhinitis patients have a bronchial reactivity without asthamatic symptoms
