Sentinel lymph node biopsy in early stage oral cavity cancers: Results of thirteen neck dissections

Klinik N oral kavite kanserlerinde boyuna yaklaşım tartışmalı bir konudur. Daha etkili yaklaşım oluşturulması amacıyla farklı protokoller ileri sürülmüştür. Bunların içerisinde son zamanlarda artan şekilde yer bulan yöntemlerden biri de sentinel lenf nodu biyopsisi yöntemidir. Ege Üniversitesi Tıp Fakultesi Hastanesi KBB Anabilim Dalında Kasım 2007-Haziran 2008 tarihleri arasında T -T oral kavite kanseri tanısıyla elektif boyun diseksiyonu endikasyonu konan 12 hastaya 13 boyun diseksiyonu, metastatik lenf nodlarının tesbiti için sentinel lenf nodu biyopsisi yöntemiyle uygulanmıştır. Saptanan lenf nodları frozen kesit yöntemi ile incelenmiş, sonuca göre boyun diseksiyonu selektif ya da kapsamlı olacak şekilde genişletilerek tamamlanmıştır. Çalışma sonucunda yöntemin negatif prediktif değeri %100, pozitif prediktif değeri %100, frozen kesit doğruluk oranı %100 ve sentinel lenf nodu tesbit oranı %100 olarak saptanmıştır. Sonuç olarak sentinel lenf nodu örneklemesi yönteminin erken evre oral kavite kanserlerinde klinik N boyuna yaklaşımda patolojik evreleme için kullanılabilir bir yöntem olduğu sonucuna varılmıştır.Management of the N neck in oral cavity cancers still remains as a controversial issue. Different methods were proposed for determining metastatic lymph nodes. Among these methods, sentinel lymph node biopsy has found its place in an increasing manner in recent years. In our Department of Otolaryngology, Ege University Hospital of Medical Faculty between November 2007 and June 2008, 13 neck dissection materials of 12 patients with T -T oral cavity cancer who were clinically N but elected for neck dissection were included in the study for detection of possible metastatic lymph nodes. Sentinel lymph nodes were sent for frozen section examination and either a selective or a comprehensive neck dissection was performed for each neck according to the results. After final histopathologic examination of the specimens, the negative predictive value, the positive predictive value, the accuracy of the sentinel lymph node biopsy and frozen section accuracy were 100%. As a conclusion, sentinel lymph node biopsy was found to be an efficient method in pathological staging and management of N neck in early T-stage oral cavity cancers
