Solitary rectal ulcer syndrome

Amaç: Soliter rektal ülser (SRÜ), genellikle pelvik taban bozuklukları ile ilgili olduğu düşünülen nadir bir hastalıktır. Bu çalışmanın amacı kliniğimizde cerrahi tedavi önerilen olguların tedavi sonuçlarını gözden geçirmektir. Yöntem: Bu çalışmada 2003–2008 yılları arasında kliniğimizde takip ve tedavileri yapılan toplam onbir olgumuzun verileri retrospektif olarak değerlendirilerek tartışılmıştır. Sonuçlar: Toplam onbir hastamızın sekizi opere edilmiştir. Cerrahi uygulanan sekiz hastadan birine saptırıcı kolostomi gerekmiştir. Bir olguya girişimden 18 ay sonra bride bağlı ince barsak obstrüksiyonu nedeni ile bridektomi uygulanması gerekmiştir. Tıbbi tedavi uygulanan üç olgunun ikisine cerrahi tedaviyi reddetmeleri nedeni ile cerrahi uygulanamamıştır. Bir hastaya cerrahinin yüksek riskli olması sebebiyle tıbbi tedavi önerilmiştir. Ortalama takip süresi 21 aydır. İki olguda pelvik ağrı rekürrensi saptanmıştır. Sonuç: Soliter rektal ülser tedavisinde, bugün için cerrahi tedavinin başarısızlık ihtimaline rağmen inatçı kanama, yaşam kalitesini bozan pelvik ağrı ve tam prolapsus olan hastalar cerrahi tedavi adayı olabilirler.Purpose: Solitary Rectal Ulcer (SRU) is a rare pathology which is usually thought to be associated with pelvic floor disorders. The aim of this study was to review the results of the patients who were recommended to be treated by surgical methods. Materials and Method: We retrospectively analyzed the eleven patients with SRU who were followed and treated between 2003-2008 in our clinic. Results: Eight out of eleven patients were treated with operative procedures. In one out of eight patients who were operated a diverting colostomy was needed. In one case a reoperation for bridectomy was needed 18 months after the first operation. Two out of three medical treatment group surgery could not be done due to patient refusal. Last one patient treated with medical therapy was because of high operative risks. Mean follow-up time was 21 months. In two patients' pelvic pain recurrences were observed. Conclusion: Consequently, although risk of failure after surgery, surgery should be performed as a first choice in patients with intractable bleeding, pelvic pain with disturbance of quality of life and total prolapsus
