Outcome study of surgical treatment for intermittent exotropia

Amaç: Intermitan ekzotropyalı olguların binoküler duyusal füzyonunu sürdürebilmesi için çeşitli tedavi seçenekleri bulunmaktadır. Biz bu çalışmada, intermitan ekzotropyalı olgularda uyguladığımız cerrahi tedavinin fonksiyonel ve kozmetik sonuçlarını değerlendirdik. Gereç ve Yöntem: İntermitan ekzotropya nedeni ile cerrahi tedavi uygulanan 42 olgunun verileri retrospektif olarak incelendi. Olguların 22’si erkek, 20’si kadın olup yaşları 3 ile 42 arasında değişmekte idi. Cerrahi ile daha iyi hale gelmiş ya da değişmeden kalmış stereopsis derecesi ile birlikte hem uzak hem de yakın için 10 PD’lik ya da daha az bir miktarda forya/ intermitan tropya başarı kriteri olarak kabul edildi. Binoküler görme Bagolini camları, Titmus testi ve sinoptofor ile değerlendirildi. Olgular cerrahi sonrası ortalama 20.8 ay (12-68 ay) takip edildiler. Bulgular: Cerrahi öncesi 30.85±9.96 PD (16 PD-60 PD arası) olan uzak ekzodeviasyon derecesinin, cerrahi tedavi sonrası son kontrolde 2.95±5.43 PD’ye (+16 PD ile -18 PD arası) düştüğü saptandı. Son kontrolde olguların 37’sinde (%88) başarı kriterine ulaşıldı. Yetersiz ve aşırı düzeltme oranlarının sırasıyla %9 ve %2 olarak gerçekleştiği saptandı. Hiçbir olguda stereopsis derecesinde bir kayıp ile karşılaşılmadı. Hiçbir olguya rezidüel ekzodeviasyon için ikinci operasyon uygulanmadı. Sadece 1 olguya, konsekütif ezotropya için ikinci cerrahi planlandı. Sonuç: İntermitan ekzotropyalı olguların çoğunluğunda cerrahi ile başarılı fonksiyonel ve kozmetik sonuçlar elde edilmiştir. Bu nedenle, uygun yapıldığında, intermittan ekzotropya tedavisinde cerrahiden çekinilmemeli ve ilk tedavi seçeneklerinden biri olarak kabul edilmelidir.Purpose: There are various treatment modalities to maintain binocular sensory fusion in intermittent exotropia. In the current study, we evaluated the functional and cosmetic outcomes of surgery in intermittent exotropia. Material and Methods: The charts of 42 cases who underwent surgery for intermittent exotropia were reviewed retrospectively. There were 22 male and 20 female patients aged from 3 to 42 years. We defined a successful surgical result as phoria/ intermittent tropia 10 PD or less for both near and distance with improved or unchanged stereopsis. Sensory status was evaluated with the Bagolini striated glasses, Titmus stereotest and synoptophore. The average postoperative follow up period was 20.8 months (ranged from 12 to 68 months). Results: Motor tests demonstrated that 37 (88%) of patients were successfully aligned by the initial procedure performed for a mean of 30.85±9.96 (16-60 PD) prism diopters of preoperative exodeviation while viewing distance targets. The mean postoperative exodeviation at the final visit was 2.95±5.43 (+16—18) prism diopters. The incidence of undercorrections and overcorrections at the final follow up examination were 9% and 2% respectively. All cases with good stereopsis preoperatively remained the same. Secondary surgery for residual exodeviation was not performed in any cases. Only in one case, secondary surgery was planned for consecutive esotropia. Conclusion: Successful functional and cosmetic results were achieved in the majority of cases treated for intermittent exotropia by surgery. Hence, surgery, when performed appropriately, would not be abstained and would be regarded as one of the first choices in the treatment of intermittent exotropia
