Evaluation mast cells in the synovium and the synovial fluid in knee pathologies

Enflamatuar reaksiyonlarındaki mediatör rolleri bilinen mast hücreleri (MH)'nin, çeşitli patolojilerde sinovya ve sinovyal sıvıda anlamlı sayısal değişiklikler gösterdiği ve bu farklılıkların, güçlük gösteren olgularda ayırıcı tanıda rol oynayabileceği düşüncesiyle, diz ekleminin enflamatuar, dejeneratif ve travmatik patolojilerinde MH'nin sinovya ve sinovyal sıvıda sayısal değişkenlikleri araştırılmıştır. 32 olguda, diz eklemine yönelik artroskopik ve açık cerrahi girişimler sırasında sinovya ve sinovyal sıvı örnekleri alınmıştır. Bu olgularda, diz eklemindeki temel patoloji; 15 olguda travmatik, 9 olguda enflamatuar, 6 olguda dejeneratif artrit olarak belirlenmiştir. Toluidin mavisi ile boyanan örneklerde ışık mikroskobu altında 10 değişik alanda MH sayısı toplamı alınmıştır. Biopsi örneklerinde, ortalama toplam MH sayısı, 34 (1-108) iken sinovyal sıvıda bunun 2 katına dek çıkmaktadır; ort. 71 (5-198). Ancak her iki örnekteki MH sayısı yönünden belirgin bir korelasyon saptanamamıştır. Dizdeki temel patoloji yönünden, her grup içinde MH sayısı çok değişkenlik göstermekte olup, gruplar arasında anlamlı bir farklılık saptanmamıştır. Yalnızca, temel nedeni ne olursa olsun, dizdeki patoloji eğer akut/aktif dönemde ise, hem sinovyada hem de sinovyal sıvıda yüksek MH değerleri elde edilmiştir. Sonuç olarak; sinovya ve sinovyal sıvıdaki MH'nin kantitatif analizi, kronik patolojilerde temel patolojinin belirlenmesi yönünden herhangi bir ayırıcı tam değeri taşımamaktadır. Ancak, nedeni ne olursa olsun akut/aktif dönemin belirlenmesinde değer taşıyabilmektedir.Purpose: Mast cells (MC) which are mediators in inflammatory reactions, increase significantly in the synovium and the synovial fluid in various pathologies. To evaluate its value in the differential diagnosis of diagnostically difficult cases, quantitative changes of MC in the synovium and the synovial fluid were studied in inflammatory, degenerative and traumatic processes of the knee joint. Material: Synovium and synovial fluid samples were obtained from 32 patients during arthroscopic or open procedures. Basic ethiological factor for the pathology in the knee joint was traumatic in 15 inflammatory in 9, and degenerative in 6 cases. Method: Samples were stained with toluidine blue after the fixation procedure and MC were counted in ten different areas under light microscopy. Results: Average MC number in the synovium was 34 (1-108), while it was 71 (5-196) in the synovial fluid. No correlation was found between the MC number of two types of samples. Also no significant difference was found among the pathologic groups. However, a statistically significant increase both in the synovium and the synovial fluid was found when the pathology in the knee joint was in the acute/ active phase of any disease regardless the basic pathology. Conclusion: In conclusion, quantitative analysis of MC in the synovium and the synovial fluid does not contribute to the differential diagnosis in chronic knee pathologies, but, can be a good indicator of disease activity
