The role of radiotherapy in the management of patients with aggressive fibromatosis

Radyoterapi Agresif Fibromatozis tedavisinde etkili bir tedavi yöntemidir. Bu çalışmada, Ocak 1991- Aralık 2005 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Radyasyon Onkolojisi Anabilim Dalı’nda agresif fibromatozis tanısı ile radyoterapi uygulanan 24 olgunun tedavi sonuçları retrospektif olarak değerlendirilmiştir. Olguların 10’u erkek, 14’ü kadın olup, tümör 12 olguda gövde, 12 olguda da ekstremite yerleşimlidir. Olguların 14’ü primer cerrahi sonrası yinelemede eksternal radyoterapi programına alınmıştır. Altı olguya ilk operasyondan sonra adjuvan radyoterapi, iki olguya neoadjuvan radyoterapi ve iki olguya da cerrahi uygulanmadan definitif radyoterapi uygulanmıştır. Medyan 62 ay (5-144) izlem sonrası; 3 olguda lokal yineleme saptanmıştır. Bu olgulardan birine salvaj cerrahi ve ikisine tamoksifen tedavisi uygulanmıştır. Neoadjuvan radyoterapi uygulanan olguların birinde radyoterapi sırasında tümör tamamen kaybolmuştur ve operasyon piyesinde canlı tümör görülmemiştir. Halen 20 olgu sağ ve hastalıksız, 3 olgu progresyon göstermeyen kalıntı tümör ile izlenirken, bir olgu başka bir nedenle kaybedilmiştir. Beş yıllık hastalıksız sağkalım %81 olarak belirlenmiştir. Radyoterapi agresif fibromatozis tedavisinde cerrahi sonrası adjuvan veya cerrahiye izin vermeyen büyük tümörü olan olgularda neoadjuvan amaçlı güvenle etkin uygulanabilen bir tedavi modelidir.Radiotherapy is one of the effective treatments for aggressive fibromatosis. Between January 1991 to December 2005, 24 patients with aggressive fibromatosis were treated by external radiotherapy at Ege University Medical School Department of Radiation Oncology and the data were analyzed retrospectively. Ten patients were men and 14 patients were women. The localization of the tumor was chest wall in 12 patients and extremity in 12 patients. Fourteen patients had recurrent disease before radiotherapy. Six patients were received adjuvant radiotherapy after the first-line surgery, two patients were referred for neoadjuvant and two were for curative radiotherapy. Within median 62 months (range 5-144) follow up, three patients developed local recurrences. One of them underwent salvage surgery, two had received medical treatment as tamoxifen. One of the patients who received neoadjuvant radiotherapy, tumor could not been detected in pathological specimen. Type of the surgery and margin status have an influence on the response of radiotherapy . Five year local recurrence free survival rate was %81. For aggressive fibromatosis, radiotherapy could be used effectively for adjuvant, neoadjuvant or curative intent
