Ranitidine bismuth citrate based dual and triple therapies in Helicobacter pylori eradication [Helicobacter pylori eradikasyonunda ranitidin bizmut sitratli ikili ve uclu tedaviler]

Most studies on Helicobacter pylori (H.pylori) eradication have been performed using a proton pump inhibitor plus antibiotic combinations. Studies with ranitidine bismuth citrate, a new combinations drug, on H.pylori eradication are very limited. To investigate the efficacy of ranitidine bismuth citrate + clarithromycin dual therapy and ranitidine bismuth citrate + amoxycillin+clarithromycin triple therapy, 65 H.pylori (+) dyspeptic patients with duodenal ulcer or non-ulcer dyspepsia were included in this study. The cases were randomly allocated to either ranitidine bismuth citrate-clarithromycin (ranitidine bismuth citrate: 400mg2xBID/14 days+clarithromycin: 500mg2xBID/14 days) or ranitidine bismuth citrate- amoxycillin clarithromycin (ranitidine bismuth citrate: 400mg 2xBID/14 days+amoxycillin: 1000mg2xB1D/1-7.days+clarithromycin: 500mg 2xBID/1-7.days) treatment groups. Fifty-seven cases attended follow up one month after the end of the therapy. H.pylori eradication was achieved in 85.7% (24/28) and 96.5% (28/29) of the patients in ranitidine bismuth citrate-clarithromycin and ranitidine bismuth citrate amoxycillin clarithromycin groups respectively (p>0.05). Mild side effects (35.7% in ranitidine bismuth citrate- clarithromycin and 31% in ranitidine bismuth citrate-amoxycillin clarithromycin group, p>0.05), mostly taste disturbance, discontinued the did not cause any patients to discontinue treatment. In conclusion, ranitidine bismuth citrate-clarithromycin and ranitidine bismuth citrate-amoxycillin clarithromycin therapies are highly efficient in H.pylori eradication. Ranitidine bismuth citrate-amoxycillin clarithromycin triple therapy, with a higher eradication rate, seems to be a particularly good alternative to proton pump inhibitor based triple therapies for H.pylori eradication
