The purpose of the study to predict individual’s risk for hospital readmission for congestive heart failure using data available at the time of index hospital discharge MATERIALS AND METHODS Patients admitted for heart failure previously in madras medical college and Rajiv Gandhi government general hospital and now readmitted with history of heart failure symptoms within the 3 months of discharge are included in the study. Total sample size is 98. The study period is April 2014 – September 2014. .
