Tuberculoma cerebral, una forma infrecuente de tumor cerebral. A propósito de un caso

Cerebral tuberculomas are extrapulmonary manifestations of tuberculosis, which may be absent or be very subtle in about 50% of patients who suffer from it. It is predominant in developing countries and currently there is an increase of it with the advent of acquired immunodeficiency syndrome. A patient who comes to the outpatient office due to late epilepsy is presented. He had no history of tuberculosis, and was diagnosed, by computerized axial tomography, a right parietal mass lesion that by its aspect seemed a brain metastasis. The surgical procedure was performed and the biopsy showed the diagnosis of cerebral tuberculoma. This is a rare case of a patient with a cerebral tuberculoma and no history of having suffered from tuberculosis. This finding led to consider surgery as a first choice, although the literature reviewed recommends an initially medical treatment.Los tuberculomas cerebrales son manifestaciones extrapulmonares de la tuberculosis, que puede estar ausente o muy sutil en cerca de un 50% de los enfermos que la padecen; es predominante en aquellos países en vías de desarrollo y en la actualidad se informa un incremento con el advenimiento del síndrome de inmunodeficiencia adquirida. Se presenta un enfermo que acude a consulta externa por epilepsia tardía, sin antecedentes previos de tuberculosis, a quien se le diagnostica, por tomografia axial computadorizada, una lesión tumoral parietal derecha que, por sus características, impresionaba una metástasis cerebral. El procedimiento quirúrgico fue realizado y la biopsia arrojó el diagnóstico de tuberculoma cerebral. Nos encontramos ante un caso poco frecuente de un paciente que padece un tuberculoma cerebral y no tenía antecedentes de tuberculosis; este dato hizo que se considerara la intervención quirúrgica como primera opción aunque la literatura revisada recomienda inicialmente el tratamiento médico
