Perfiles de resistencia antimicrobiana de Staphylococcus aureus aislados a partir de leche de vacas ubicadas en tambos de la Provincia de Buenos Aires

Bovine mastitis is the infectious disease with the greatest impact on world dairy production. In Argentina, Staphylococcus aureus (S. aureus) is the most important pathogen in terms of prevalence and pathogenicity. It can resist antimicrobial treatments through different mechanisms. Currently, there is great concern about antibiotic resistance, both in human medicine and animal health. In our country, there is scarce information about antimicrobial resistance on bovine mastitis strains. The main objective of the project was to evaluate the in Vitro susceptibility of S. aureus isolated from milk samples obtained in dairy farms located in Buenos Aires. Between 2015 and 2016, 109 isolates of milk samples were obtained from cows with clinical mastitis, subclinical and bulk tank milk samples. Until July 2016, the susceptibility of 44 isolates were evaluated following the recommendations of the Clinical and Laboratory Standards Institute (CLSI). The antimicrobial agents included in the study were selected consistent with their importance in the treatment of infectious diseases caused by S. aureus and the detection of the different phenotypes of resistance. We have found that Amoxicillin-Ac. Clavulanic, Ciprofloxacin and Cephalotin have a percentage of susceptibility of 100 %. The percentages of sensitivity to the remaining antimicrobials were lower than 100 %, indicating resistance against Rifampicin (5 %), Ampicillin (9 %), Nalidixic Acid (27 %), Penicillin (34 %), Erythromycin Clindamycin (57 %), Vancomycin (57 %), Tetracycline (59 %) and Oxacillin (61 %). Currently, the level of resistance to certain agents exceeds expectations and opens new questions. Resistance to oxacillin is an indicative sing of Methicillin-Resistance and should be specifically studied through further studies. Compared with similar studies carried out in Argentina, the percentage of resistance found with some of the antimicrobial agents is high (Vancomycin, tetracyclines, quinolones, macrolides, and lincomycins). This data will be analyzed to detect specific resistance-phenotypes and their association with specific molecular mechanisms. Among the isolates that were evaluated, 7 % showed resistance against a single antibiotic, 64 % were resistant to two or more antibiotics (multi drug resistance) and 29 % showed no resistance.La mastitis bovina es la enfermedad infecciosa de mayor impacto en la producción lechera mundial. En Argentina, el Staphylococcus aureus (S. aureus) es el patógeno de mayor prevalencia y patogenicidad, capaz de resistir a los tratamientos antimicrobianos mediante diferentes mecanismos. Actualmente, existe una gran preocupación en relación a la antibioticorresistencia, tanto en medicina humana, como en salud animal. En nuestro país, los estudios realizados sobre cepas de S. aureus causantes de mastitis bovina son relativamente escasos. El objetivo del presente trabajo fue evaluar la susceptibilidad in vitro de S. aureus aislados de muestras lácteas provenientes de establecimientos lecheros de la Provincia de Buenos Aires. Entre el 2015 y 2016 se obtuvieron 109 aislamientos de muestras de leche de vacas con mastitis clínica, subclínica y leche de tanque de refrigeración. Hasta julio de 2016, se evaluó la susceptibilidad de 44 aislamientos siguiendo las recomendaciones del Clinical and Laboratory Standards Institute (CLSI). Los agentes antimicrobianos fueron seleccionados por su importancia en el tratamiento de infecciones causadas por S. aureus y en la detección temprana de fenotipos de resistencia. Los resultados obtenidos indican que la sensibilidad frente a Amoxicilina-Ac. Clavulanico, Ciprofloxacina y Cefalotina fue del 100 %. Los porcentajes de sensibilidad frente a los restantes antimicrobianos fueron menores al 100 %, e indicaron resistencia frente a Rifampicina (5 %), Ampicilina (9 %), Ácido Nalidíxico (27 %), Penicilina (34 %), Eritromicina (57 %), Clindamicina (57 %), Vancomicina (57 %), Tetraciclina (59 %) y Oxacilina (61 %). A la fecha, el nivel de resistencia frente a determinados agentes supera lo esperado y abre nuevos interrogantes. La resistencia a la Oxacilina es indicador de Meticilino-Resistencia y debe estudiarse de manera específica mediante nuevos estudios. En comparación con estudios similares realizados en nuestro país, el porcentaje de resistencia hallado frente a otros antimicrobianos (vancomicina, tetraciclinas, quinolonas, macrólidos y lincomicinas) es elevado. A partir de estos datos se estudiarán los fenotipos de resistencia y su asociación con mecanismos moleculares específicos. Del total de aislamientos evaluados, el 7 % presentó resistencia frente a un solo antibiótico, el 64 % frente a dos o más antibióticos (multirresistencia) y el 29 % no presentaron resistencia alguna
