Paediatric‐onset inflammatory bowel disease (PO‐IBD) is associated with greater morbidity compared to adult‐onset IBD. However, as not all children with PO‐IBD will have poor outcome and the best management decisions involve weighing risks versus benefit and wishes of patient's and family, we review risk factors of IBD progression in children and summarise rapidly expanding treatment choices, potential drug‐related adverse events and risk minimisation strategies, ending with new treatment paradigms focusing on long‐term goal of intestinal healing.
