Noves estratègies per la millora de les característiques biològiques dels implants dentals

Despite the success rates of dental implants, even nowadays, the risk of implant failure is still relevant, especially in long term periods. The main causes reported for this failure are the lack of osteointegration with the surrounding bone and the appearance of a biofilm that causes periimplantitis and therefore the loosening of the implant. To prevent infections related to dental implants, one possible strategy is the detection of the presence of bacteria in the initial stages of biofilm growth. Among all detection methods, impedimetric biosensors, which are cheap and easy to use, are the most promising tool in this field. For the design of the biosensor, antimicrobial peptides (AMPs), which are well-known components of the immune system and show a broad range of antibacterial activity, can be used to develop very effective biorecognition elements. Another approach, is to create antibacterial surfaces, to reduce bacterial colonization or to prevent its adhesion, by anchoring biomolecules with antibacterial properties. Regarding this, surface biofunctionalization of dental implants is a method that is also widely used to improve the interactions between cells and material and thus, improve the other problem found in dental implants: the lack of osteointegration. There are many methods to modify the surfaces of the implants and their interaction with the tissues, but chemical strategies based on the immobilization of organic biomolecules, such as peptide motifs, have shown very good results both in vitro and in vivo. Therefore, in this Thesis, two strategies are described to improve the viability of dental implants: a first one focused on the design of a biosensor for the detection of bacteria at an early stage of the biofilm development process; and a second that focuses on the improvement of the surface of the dental implant by means of the anchorage of organic biomolecules with multifunctionality.El Titani (Ti) i els seus aliatges són actualment els materials escollits per moltes aplicacions dentals, degut sobretot a la seva òptima biocompatibilitat i les seves excel·lents propietats mecàniques. Tot i les taxes d'èxit dels implants dentals, encara avui en dia, el risc de fallida de l'implant encara és rellevant, sobretot a llarg termini. Les principals causes reportades per aquesta fallida són la falta d'osteointegració amb l'os circumdant i l'aparició d'un biofilm que provoca la peri-implantitis i per tant, l'afluixament de l'implant. Per prevenir les infeccions relacionades amb implants dentals, una estratègia és la detecció de la presència de bacteris a les fases inicials del creixement del biofilm. D'entre tots els mètodes per a la detecció que existeixen, els biosensors impedimètrics, els quals són econòmics i fàcils d'usar, són l'eina més prometedora en aquest camp. Pel disseny del biosensor els pèptids antimicrobians (AMPs), que són components reconeguts del sistema immunitari i mostren un rang molt ampli d'activitat antibacteriana, es poden usar per desenvolupar elements de bioreconeixement molt efectius. Una altra aproximació és crear superfícies antibacterianes, per reduir l'adhesió dels bacteris o bé evitar-ne la seva adhesió, mitjançant l'ancoratge de biomolècules amb propietats antibacterianes. Seguint aquesta línia, la biofuncionalització de superfícies dels implants dentals és un mètode molt usat també per millorar les interaccions entre cèl·lules i material i millorar així l'altre problema que trobem en els implants dentals: la falta d'osteointegració. Existeixen molts mètodes per modificar les superfícies dels implants i la seva interacció amb els teixits, però les estratègies químiques basades en la immobilització de biomolècules orgàniques, com motius peptídics, ha demostrat uns molt bons resultats tant in vitro com in vivo. Per tant, en aquesta Tesis es descriuen dues estratègies per millorar la viabilitat dels implants dentals: una primera enfocada en el disseny d'un biosensor per a la detecció de les bactèries en una fase inicial del procés de desenvolupament de biofilm; i una segona que es focalitza en la millora de la superfície de l'implant dental mitjançant l'ancoratge de biomolècules orgàniques amb diferents funcionalitats.Postprint (published version
