Background: Adenine phosporibosyltransferase (APRT) deficiency is a metabolic disorder which is the cause of adenine oxidation to 2,8-dihydroxyadenine (DHA) instead of normal conversion to AMP. DHA is poorly soluble at physical level and therefore precipitates in urine and can cause of acute renal failure. The symptoms have been treated with allopurinol which converts to its active metabolite oxypurinol quickly after it enters the body. It is important to being able to quantify both oxypurinol and DHA in plasma for better diagnosis and treatment. Objective: Develop a quantification method with Ultra Performance Liquid Chromatography with tandem mass spectrometry (UPLC-MS/MS). In addition to sample preparation method development for DHA and oxypurinol quantification in plasma. Methods: Design of experiment (DOE) methodology was implemented to optimize UPLC-MS/MS assay for quantification of oxypurinol. Sample preparations methods with solid phase extraction (SPE) and protein precipitation (PPT) were developed for sample cleanup of plasma samples prior to quantification of DHA and oxypurinol. Results: UPLC-MS/MS assay was developed and used for quantifying oxypurinol in plasma samples from patients suffering from APRT deficiency. Sample preparation methods for cleanup of plasma samples for quantification of DHA and oxypurinol. Protein precipitation produced the best results for both compounds. Conclusions: Event though the optimized UPLC-MS/MS assay was used for quantifying oxypurinol in patient samples from patients suffering from APRT deficiency, the method needs validating for more accurate quantification.
