Trends of antibiotic prescribing among inpatients of medicine departments in two private sector hospitals over 9 years; one nonteaching and one teaching hospital in Central India

Bakgrunnur: Rannsóknir byggðar á staðbundnum gögnum eru nauðsynlegar til að framkvæma viðeigandi inngrip á ávísunum sýklalyfja og takmarka sýklalyfjaónæmi. Þörf er á fleiri rannsóknum frá einkareknum sjúkrahúsum í Indlandi þar sem einkareknar heilbrigðisstofnanir eru um 80% af heilbrigðisstofnunum á Indlandi, en fyrrum rannsóknir hafa verið frá ríkisreknum sjúkrahúsum. Markmið: Kanna mynstur á ávísunum fyrir sýklalyf á lyflækningadeildum yfir níu ára tímabil á tveimur sjúkrahúsum, öðru sem er ekki kennslusjúkrahús (NTH) og hinu sem er kennslusjúkrahús (TH), í Mið-Indlandi Aðferðir: Hlutföll sýklalyfjaávísana voru greind með tilliti til kyns og aldurs. ATC/DDD flokkunarkerfið frá alþjóðastofnun var notað til að setja fram ávsanir sýklalyfja sem DDD/100 rúmdagar. Algengustu sýklalyfjahóparnir sem voru ávísaðir voru greindir niður í fjórða stig af ATC flokkunarkerfinu. Breyting á ávísunum sýklalyfja í heild sinni í DDD/100 rúmdagar var greind á milli mánaða og ára, og á milli ára fyrir sérstaka sýklalyfjahópa. Niðurstöður: Algengara var að ávísa sýklalyfjum til karlmanns sjúklinga á lyflækningadeildinni á NTH. Magn sýklalyfjaávísana í heild sinni hækkaði á NTH en lækkaði á TH á níu árum. Á báðum sjúkrahúsunum var algengt að ávísa þriðju kynslóð cefalósporína, flúórókínólónum, blöndu penicillina og Ímídazólafleiðum. Á NTH var algengt að ávísa blöndum bakteríulyfja en tetracýklínsamböndum á TH. Mesti munurinn á ávísunum sýklalyfja yfir níu árin á milli sjúkrahúsanna var fyrir flúórókínólóna þar sem ávísunum hækkaði í NTH en lækkaði í TH. Ályktanir: Ávísun sýklalyfja var mismunandi á milli sjúkrahúsanna þar sem þær jukust yfir níu árin á NTH og voru hærri á NTH en á TH. NTH ávísaði meira af blöndum af sýklalyfjum en TH. Engin stefna var á NTH um heimsóknir frá lyfjakynnum frá lyfjafyrirækjum gæti hafa skýrt þennan mun þar sem læknarnir gætu verið undir pressu að ávísa meira af sýklalyfjum og sérstaklega blöndum af sýklalyfjum.Background: Interventions on antibiotic prescribing have been connected to improve rational prescribing of antibiotics and limit the occurrence of antibiotic resistance. Researchers based on local data are needed to make the appropriate interventions on antibiotic prescribing. Studies are needed in private hospitals in India where the private sector accounts for about 80% of healthcare in India. Aim: Explore antibiotic prescribing patterns and trends in medicine departments over nine years period at two tertiary care hospitals, one non-teaching hospital (NTH) and one teaching hospital (TH), in Ujjain, India. Methods: antibiotic prescribing rates were analysed by age and gender at the hospitals. The Anatomical Therapeutic Chemical (ATC) classification / defined daily dose (DDD) system from WHO Collaborating Centre for Drug Statistics Methodology was used to present the antibiotic prescribing in DDD/100 bed days. The most commonly prescribed antibiotic groups were presented by 4th level of the ATC classification system. General trend of the overall antibiotic prescribing in DDD/100 bed days was analysed by years and months and by years for the most commonly prescribed antibiotic groups. Results: More male patients were prescribed antibiotics at the medicine department at the TH. The overall antibiotic prescribing trend increased in the NTH but decreased in the TH over the period. Third-generation cephalosporins, fluoroquinolones, combination of penicillins and imidazole derivatives were commonly prescribed at both hospitals and combinations of antibacterials at the NTH and tetracyclines at the TH. The most difference in antibiotic prescribing trend between the hospitals was for fluoroquinolones where the trend increased at the NTH but decreased at the TH. Conclusions: The antibiotic prescribing differed between hospitals where it increased over the nine years at the NTH and was higher than in the TH. The NTH prescribed more fixed dose combinations than the TH. The lack of a policy guideline about visits from pharmaceutical representatives might explain this difference where the physicians at the NTH might be under pressure to prescribe more antibiotics and especially fixed dose combinations.Erasmus
