Factores de riesgo de infarto agudo del miocardio en pacientes con diagnostico de hipertención arterial

Introduction: the acute coronary disease is the main cause of cardiovascular death and its contribution to morbidity is high.Objective: to identify the influence of several risk factors of acute myocardial infarction in the presence of this disease.Method: it was performed an observational, analytical and case- witness study in patients of the Medical Center of Integral Diagnosis ¨Secundino Urbina¨ from Coro’s municipality, Falcon State, in the Bolivarian Republic of Venezuela, during the period of January 1st, 2008 to December 31st of 2010. The patients were taken at random in the proportion 1:1 (70 cases and 70 witnesses). There were studied the socio demographic variables: alcoholism, sedentary life style, obesity, hypercholesterolemia, smoking habit and hypertension, and it was performed an unvaried strategy and a multivariate study through the statistic calculation of Wald. It was also applied the Hosmer- Lemeshow test to evaluate the adjustment of the model with the statistical package SPSS, version 15.Results: 65 years or older, belonging to the male sex increased almost 5 times more (OR 4, 6 IC95% 2, 20 – 10, 42 y OR 4, 7 IC95% 2, 7 – 10, 39) the probability of myocardial infarction. The sedentary life style, obesity, and hypercholesterolemia were the risk factors of the disease with a greater incidence in the sedentary life style; the time of evolution, greater or equal to 15 years increased the risk more than 15 times (OR 15, 49 IC95% 6, 24 – 39, 43). The risk of appearance is about 6.23 times more with a hypertension of a severe degree like the greater weight.Conclusions: the greatest degree of independence like a risk factor for the acute myocardial infarction in patients with hypertension is represented by the severe degree of hypertension, the time of evolution of the degree and the age.Introducción: la enfermedad coronaria es la causa principal de muerte cardiovascular y su contribución a la morbilidad es alta. Objetivo: identificar la influencia de diversos factores de riesgo de infarto agudo del miocardio para la ocurrencia de esta enfermedad en pacientes hipertensos. Método: Se realizó un estudio observacional, analítico, de casos y testigos, en pacientes del Centro Médico de Diagnóstico Integral Secundino Urbina, del municipio Coro, estado Falcón, de la República Bolivariana de Venezuela, desde el 1 de enero del 2008 al 31 de diciembre del 2010. Los pacientes se seleccionaron aleatoriamente en proporción 1:1 (70 casos y 70 testigos). Se estudiaron como variables los factores sociodemográficos, alcoholismo, sedentarismo, obesidad, hipercolesterolemia, hábito de fumar e hipertensión arterial y se realizó una estrategia univariada y seguidamente un estudio multivariante mediante el cálculo del estadístico de Wald. Se aplicó también la prueba de Hosmer-Lemeshow para evaluar la bondad de ajuste del modelo con el paquete estadístico SPSS versión 15. Resultados: tener 65 años o más y pertenecer al sexo masculino incrementó casi cinco veces (OR 4,6 IC95% 2,20 – 10,42 y OR 4,7 IC95% 2,7 – 10,39) más la probabilidad de aparición del infarto agudo del miocardio, la presencia de sedentarismo, hipercolesterolemia y obesidad constituyeron factores de riesgo de aparición de la enfermedad, con mayor incidencia del sedentarismo, el tiempo de evolución mayor o igual a quince años incrementó más de quince veces (OR 15,49 IC95% 6,24 – 39,43) el riesgo; se destaca, que el riesgo de ocurrencia es 6,23 veces más al tener una hipertensión arterial de grado muy severo como el de mayor peso.Conclusiones: el mayor grado de independencia como factor de riesgo para el infarto agudo del miocardio en pacientes con hipertensión arterial está representado por el grado de severidad de la hipertensión, el tiempo de evolución de la enfermedad y la edad
