Jinekolojik Kanserli Hastalarda Evde Bakım Hizmetinin Yaşam Kalitesine Etkisi

This paper covers a random intervened case control study performed to evaluate any possible effects of the home care service on the life quality of the gynecological cancer patients. The sampling covers total 70 women, including 35 ones in each of the intervention and control groups, who underwent gynecological cancer treatment at the Zekai Tahir Burak Woman Health Training and Research Hospital under the Turkish Ministry of Health. The intervention and control groups, the Questionnaire, Life Quality Scale/Cancer Patient, Golombok-Rust Inventory of Sexual Satisfaction (GRİSS) and Visual Analog Skale were applied to collect data. The nursing care service was rendered to the women of intervention group in the oncology clinic, where a number of discharge training courses took place, followed by three postdischarge home visits in the 1st, 6th and 12th weeks in addition to two phone calls in the 2nd and 3rd weeks for the ongoing care and consulting services. The control group women were known in the oncology service unit, followed by two postdischarge home visits in the 1st and 12th weeks. The control group women took no care or consultancy. The data evaluation was based on the percentage rating, Chisquare, Eisher Exact Test, Mc-Nemar Test, ANOVA. The obtained results; The control group women were found to experience problems twice higher than the intervention group (p<0,05). During the entire course of monitoring the intergroup difference was significant as of the hopelessness, fatigue, lack of feminine feeling (p<0,05). The point averages of total life quality of the control group women were low level than those of the intervention group, the point averages of GRİSS of the control group women high level than those of the intervention group and the difference between two groups was statistically significant (p<0,05). In line with the obtained results, it is suggested to make home visits for taking actions to improve the life quality and provide sexual consultancy.Araştırma, jinekolojik kanserli hastalarda evde bakım hizmetinin yaşam kalitesine etkisini belirlemek amacıyla yapılan müdahaleli randomize vaka kontrol çalışmasıdır. Araştırmanın örneklemine, T.C. Sağlık Bakanlığı Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesi?nde jinekolojik kanser tedavisi alan 35 müdahale, 35 kontrol grubu olmak üzere toplam 70 kadın alınmıştır. Veriler, her iki grupta; Görüşme Formu, Ev ziyaret izlem formu, Yaşam Kalitesi Ölçeği/Kanser Hastası, Golombok-Rust Cinsel Doyum Ölçeği (GRCDÖ) ve Ağrı değerlendirmede Görsel Analog Skala aracılığı ile toplanmıştır. Müdahale grubundaki kadınlara hastanede pre-operatif ve post-operatif dönemlerde hemşirelik bakım hizmeti verilmiş, taburculuk sonrası 1., 6. ve 12. haftalarda üç ev ziyareti, 2. ve 3. haftalarda iki telefon görüşmesi yapılarak bakım ve danışmanlık hizmetleri sürdürülmüştür. Kontrol grubunda ise; onkoloji servisinde tanışılmış, 1. ve 12. haftalarda iki ev ziyareti yapılmış ve herhangi bir bakım ya da danışmanlık hizmeti verilmemiştir. Verilerin değerlendirilmesinde; yüzdelik sayılar, ki-kare, Fisher?s Exact test, McNemar?s test, ANOVA kullanılmıştır. Araştırmadan elde edilen sonuçlara göre; taburculuk sonrası ilk on iki haftalık dönemde evde, kontrol grubundaki kadınların müdahale grubuna göre iki kat daha fazla sorun yaşadıkları belirlenmiştir (p<0,05). Umutsuzluk, ağrı, yorgunluk, kadın gibi hissetmeme ile ilgili sorun yaşama bakımından gruplararası fark tüm izlem dönemleri arasında önemli bulunmuştur (p<0,05). Kontrol grubundaki kadınların toplam yaşam kalitesi puan ortalamalarının müdahale grubuna oranla düşük, GRCDÖ puanlarının ise yüksek olduğu ve gruplararası farkın önemli olduğu belirlenmiştir (p
