Health literacy of parents of pediatric surgical patients: a correlational analysis

Despite its importance, health literacy, parental anxietyand need-for-information preoperatively, have not been sufficientlystudied in pediatric surgery. Low educational level, minorities and lowsocioeconomic level have been correlated with lower levels of healthliteracy. Parental anxiety has been related to health literacy, sex,education and information needs.Aim : The aim of this study is to investigate health literacy levels ofparents or guardians of pediatric surgical patients, their anxiety beforeconsultations of pediatric surgery, their need-for-informationpreoperatively and their preferences for participation in decision making,and their correlations with clinical and socioeconomic data. Finaly, 2tools for measuring health literacy levels will be compared in this specialpopulation.Materials and Methods : The study was conducted from December2016 to July 2018, at the outpatient pediatric surgery clinic and theinpatient ward of the 1st and 2nd Pediatric Surgical Departments of AgiaSophia Children’s Hospital in Athens, Greece. The participants (parentsor guardians) were asked to fill in a standardised anonymousquestionnaire after providing informed consent. Demographic andsocioeconomic data of the participants were recorded alng withdemographic and clinical data of the patients. Moreover, usingappropriate and validated tools the following were measured : healthliteracy levels of the participants (HLS-EU-Q16 and NVS), parentalanxiety and need-for-information preoperatively (AmsterdamPreoperative Anxiety and Preoperative Information Scale) and theirpreferences regarding decision making about their child’s proposedtreatment (Control Preferences Scale).For all analyses, significance was accepted at p < 0.05. Statisticalanalyses were performed using SPSS Statistics for Macintosh (Version24.0. Armonk, NY: IBM Corp.).Results: 55,8 % of the 1000 participants in total had adequatehealth literacy, as measured by HLS-EU-Q16, while 36,8 % presentedproblematic health literacy levels and 7,4 % inadequate. Multipleregression analysis results showed that health literacy was associatedwith nationality (β = -2,180, p <0,001), insurance status (β = -0,461, p =0,016), educational level (β = 0,075, p < 0,001) and being a healthprofessional (β = 1,791, p <0,001).Total parental anxiety scores of the sample (n=664) ranged from 4to 20 in APAIS scale, with mean (± SD) 13,99 (± 3,95). Interestingly,79,8 % of the participants were classified as being anxious, with anAPAIS score of 11 or more. The need-for-information score of parentsor guardians at the outpatient pediatric surgical unit ranged from 2 to 10with mean (± SD) 8 (± 1,88). Parental or guardian anxiety was foundassociated at the multiple regression analysis with parental healthliteracy level (β=-0.277, p<0.001), educational level (β=-0.685,p=0.002), the severity of the condition of the child (β=0.47, p=0.031),and the need-for-information preoperatively (β=0.908, p<0.001). Womenwere found to be more anxious than men (β=1.573, p<0.001).Regarding preferences about participation in decision makingconcerning the proposed treatment from the pediatric surgeon, the mostfrequent choice of the participants was sharing responsibility for thedecision with the doctor (34,2 %). Passive collaboration or delegation ofthe responsibility for the decision completely to the doctor werepreferred by the majority of the participants (44,6 %), suggesting thatalmost half of parents or guardians do not wish to be actively engagedin decision making concerning the treatment of their child. Need-forinformation(p<0,01), educational level (p<0,05), sex (p<0,05) andnationality (p<0,05) were found to be predictors of participation desire indecision making in the studied population.Concerning the comparison of health literacy measurement tools(HLS-EU-Q16 and NVS), the following was found in the studiedpopulation (n=336) : If a parent or guardian is classified as being ofadequate health literacy according to NVS, the 100% is also classifiedas such in the HLS-EU-Q16 tool. Moreover, only 5% of parents orguardians that are classified as being of adequate health literacy by theHLS-EU-Q16 tool, are classified by the NVS in a lower health literacycategory. This is due to a known ceiling effect of the HLS-EU-Q16 tool,much less present in this tool than other corresponding ones. Whenevaluating the scores as a continuous variables rather than categorical,a better correlation between them is found than in the generalpopulation (Spearman rho 0,408).Conclusions: Almost half of parents or guardians of pediatricsurgical patients presented problematic or inadequate health literacylevels. The majority of the participants present high levels of anxiety,while preferring passive roles or shared decision making with thepediatric surgeon concerning the proposed treatment. Health literacymeasurement tools are reliable on this specific population. Healthliteracy is important and should not be neglected during examiningchildren of caring for them in the pediatric surgical setting.Παρά τη σημαντικότητά τους, η εγγραμματοσύνη σεθέματα υγείας, το άγχος των γονέων ή φροντιστών και η επιθυμίαενημέρωσής τους προεγχειρητικά, δεν έχουν μελετηθεί επαρκώς σταπλαίσια της παιδοχειρουργικής. Τοάγχος των γονέων ή φροντιστών έχει συσχετισθεί με την εγγραμματοσύνησε θέματα υγείας, το φύλο, το εκπαιδευτικό επίπεδο και την επιθυμίαενημέρωσης προεγχειρητικά.Σκοπός: Σκοπός της μελέτης ήταν η μέτρηση των επιπέδωνεγγραμματοσύνης των γονέων ή φροντιστών παιδοχειρουργικών ασθενώνκαι του άγχους τους στα πλαίσια της παιδοχειρουργικής εξέτασης, ηδιερέυνηση της επιθυμίας ενημέρωσής τους προεγχειρητικά και τηςεπιθυμίας συμμετοχής τους στη λήψη αποφάσεων σχετικά με τιςπροτεινόμενες θεραπευτικές παρεμβάσεις από τον παιδοχειρουργό,καθώς και η συσχέτισή τους με κλινικά και κοινωνικοοικονομικά στοιχεία.Σκοπός επίσης ήταν η σύγκριση 2 κλιμάκων μέτρησης των επιπέδωνεγγραμματοσύνης σε θέματα υγείας στον ειδικό αυτό πληθυσμό.Υλικό και Μέθοδος: Η μελέτη πραγματοποιήθηκε από τοΔεκέμβριο του 2016 έως και τον Ιούλιο του 2018, χρησιμοποιώντας δείγμαευκολίας από τα τακτικά εξωτερικά ιατρεία και την πτέρυγα νοσηλείας τωνΑ΄και Β΄Παιδοχειρουργικών κλινικών του Νοσοκομείου Παίδων «Η ΑγίαΣοφία», στην Αθήνα. Οι συμμετέχοντες γονείς ή φροντιστές κλήθηκαν νασυμπληρώσουν ένα ανώνυμο τυποποιημένο ερωτηματολόγιο μετά τηνπαροχή συναίνεσης κατόπιν ενημέρωσης. Έγινε καταγραφή τωνδημογραφικών και κοινωνικοοικονομικών χαρακτηριστικών τωνσυμμετεχόντων, αλλά και δημογραφικών και κλινικών δεδομένων τωνμικρών ασθενών. Επίσης, με τη χρήση κατάλληλων εργαλείων προσδιορίστηκαν : τα επίπεδα εγγραμματοσύνης των συμμετεχόντων(HLS-EU-Q16 & NVS), το άγχος τους και η επιθυμία ενημέρωσής τουςπροεγχειρητικά (Amsterdam Preoperative Anxiety and PreoperativeInformation Scale) και η επιθυμία συμμετοχής τους στη λήψηθεραπευτικών αποφάσεων για το παιδί τους (Control Preferences Scale).Για όλες τις αναλύσεις, η στατιστική σημαντικότητα ορίστηκε σεεπίπεδο p<0,05. Οι στατιστικές αναλύσεις πραγματοποιήθηκανχρησιμοποιώντας το SPSS Statistics για Macintosh (Version 24.0.Armonk, NY: IBM Corp.).Αποτελέσματα: Το 55,8% των 1000 συμμετεχόντων συνολικάπαρουσίασε επαρκή εγγραμματοσύνη σε θέματα υγείας, όπως μετρήθηκεαπό το HLS-EU-Q16, ενώ το 36,8% προβληματικά επίπεδαεγγραμματοσύνης σε θέματα υγείας και το 7,4% ανεπαρκή. Τααποτελέσματα της ανάλυσης πολλαπλής παλινδρόμησης δείχνουν ότι ηεγγραμματοσύνη σε θέματα υγείας συσχετίζεται με την εθνικότητα (β = -2,180, p <0,001), το είδος ασφάλισης υγείας των συμμετεχόντων (β = -0,461, p = 0,016), το μορφωτικό επίπεδο των συμμετεχόντων (β = 0,075, p< 0,001) και την ιδιότητα του επαγγελματία υγείας (β = 1,791, p <0,001).Η βαθμολογία συνολικού άγχους του δείγματος (n=664)κυμαίνονταν μεταξύ 4 και 20 στην κλίμακα APAIS, με μέση τιμή (± SD)13,99 (± 3,95). Είναι ενδιαφέρον το γεγονός ότι το 79,8% τωνσυμμετεχόντων ταξινομείται ως υψηλού άγχους, εμφανίζοντας σκορμεγαλύτερο ή ίσο του 11. Το σκορ επιθυμίας ενημέρωσης των γονέων ήφροντιστών στα τακτικά εξωτερικά παιδοχειρουργικά ιατρεία κυμάνθηκεμεταξύ 2 και 10 και η μέση τιμή ± SD ήταν 8 ± 1,88. Το άγχος των γονέωνή φροντιστών βρέθηκε να σχετίζεται αρνητικά με το σκορεγγραμματοσύνης σε θέματα υγείας (β = -0,277, p <0,001) και το επίπεδοεκπαίδευσης των γονέων ή φροντιστών (β = -0,685, p = 0,002). Αντίθετα,βρέθηκε να συνδέεται θετικά με τη βαρύτητα της χειρουργικής παθολογίαςτου παιδιού (β = 0,47, p = 0,031) και την επιθυμία για ενημέρωση τωνγονέων ή φροντιστών προεγχειρητικά (β = 0,908, ρ <0,001). Επιπλέον, οι γυναίκες βρέθηκαν περισσότερο αγχωμένες από τους άνδρες (β = 1.573, p<0.001).Σχετικά με την επιθυμία συμμετοχής στη λήψη αποφάσεων για τηνπροτεινόμενη από τον παιδοχειρουργό θεραπεία, η πιο συχνή επιλογή τωνσυμμετεχόντων ήταν η συναπόφαση με το γιατρό (34,2 %). Η παθητικήσυμμετοχή ή συνεργασία με τον παιδοχειρουργό και ο αποκλειστικάπαθητικός ρόλος προτιμήθηκαν από την πλειοψηφία των συμμετεχόντων(44,6 %), υποδεικνύοντας ότι σχεδόν οι μισοί γονείς ή φροντιστές δενήθελαν να συμμετέχουν ενεργά στη λήψη αποφάσεων σχετικά με τηθεραπεία του παιδιού τους.
