Comparative evaluation of clinical aspects in post-operative inguinal herniorrhaphy by laparotomy and laparoscopy

The inguinal hernia is the most frequent among the hernias of the abdominal wall. Different surgical techniques were developed for this condition and, despite the studies already done, comparing the postoperative of these techniques, there is still no consensus of which has a lower morbidity. The objective was to identify the clinical differences in the postoperative period of inguinal herniorrhaphy laparotomy and laparoscopic. This is an observational cross-sectional study, carried out in the city of Florianópolis - Santa Catarina. A total of 150 records were analyzed, 75 of which were laparotomy herniorrhaphy and 75 were laparoscopic. The data were collected in electronic medical records, tabulated in the Windows Excel software and later analyzed by the SPSS (Version 18.0 program. [Computer program]. Chicago: SPSS Inc). In the evaluation of the postoperative clinical aspects, comparing accesses, laparotomy and laparoscopic, it was observed that postoperative pain was reported by 64% of the patients in laparotomy and in 6.7% in the laparoscopic. Complications (surgical wound infection, hematoma and seroma) were present in 14.7% of the laparotomy herniorrhaphy and 4.0% of the laparoscopic. The time to return to work was less or equal to 30 days in 94.7% of the patients submitted to the laparoscopic and in 53.3% of the patients submitted to laparotomy. It can observed that the approach that includes less postoperative pain, shorter time to return to work and lower rates of complications for inguinal herniorrhaphy was the laparoscopic. Diversas técnicas cirúrgicas foram desenvolvidas para esta afecção e, apesar dos estudos já realizados, comparando o pós-operatório dessas técnicas, ainda não há um consenso de qual possui menor morbidade. O objetivo deste estudo foi identificar as diferenças clínicas do pós-operatório da herniorrafia inguinal por laparotomia e laparoscopia. Trata-se de um estudo observacional analítico transversal, realizado na cidade de Florianópolis - Santa Catarina. Foram analisados 150 prontuários, sendo 75 de herniorrafia por laparotomia e 75 por laparoscopia. Os dados foram coletados em prontuários eletrônicos, tabulados no software Windows Excel e, posteriormente, analisados pelo programa SPSS (Version 18.0. [Computer program] Chicago: SPSS Inc; 2009). Na avaliação dos aspectos clínicos pós-operatórios comparados entre as vias de acesso, laparotomia e laparoscopia, observou-se que a dor na consulta pós-operatória foi relatada em 64% dos pacientes na laparotomia e em 6,7% na laparoscopia. As complicações (infecção da ferida operatória, hematoma e seroma) estiveram presentes em 14,7% das herniorrafias por laparotomia e em 4,0% das laparoscopias. O tempo de retorno ao trabalho foi menor ou igual a 30 dias em 94,7% dos pacientes submetidos laparoscopia e em 53,3% dos submetidos a laparotomia. Portanto, constatou-se que a via que contempla menor dor pós-operatória, menor tempo de retorno ao trabalho e menor índice de complicações para herniorrafia inguinal foi a laparoscópica.
