Perfil psicológico de pacientes candidatos à cirurgia bariátrica

Background: Bariatric surgery has shown to be a good option for the obesity treatment and its comorbidities. The psychological preoperative interview of the patient who is undergoing this procedure is important so the obesity treatment can be successful and can maintain the results on the long-term. Aim: Characterize the psychological profile of bariatric surgery candidates from the preoperative psychological interview. Methods: A cross-sectional observational study. A total of 411 psychological preoperative interviews were analyzed, in a private practice of southern Santa Catarina, between the years of 2011 and 2016. Results: Amongst all the psychological interviews that were analyzed, 311 (75,1%) were women, medin age was 36 years old. The abuse of substances was found in 66 (15,9%) of the interviews, being nicotine (8,7%) and alcohol (4,1%) the most used substances. 26,6% of the interviewed patients referred to anxiety and 5,1% referred depression. 17,4% of the patients had a high score at the BITE (Bulimic Investigatory test of Edinburgh) – characterizing the presence of bulimia and 25,6% showed eating pattern as not usual, but there wasn’t bulimia. Women showed more chances of having bulimia (OR= 2,642; IC95%:1,288 a 5,420; p=0,007).The higher chance of having bulimia also happened in patients who had a current diagnosis of a psychiatric disease (OR=2,351; IC=1,275 a 4,335; p=0,006). Who had anxiety also had higher chances of having bulimia (OR=1,971; IC=1,70 a 3,631; p=0,029). Conclusion: Most of the candidates of bariatric surgery are women, which have higher odds of having bulimia. Depression and anxiety are the psychiatric diseases most common in this population. A avaliação psicológica pré-operatória do paciente que se submeterá a tal procedimento é relevante para que se tenha sucesso no tratamento da obesidade, e para a sua manutenção em longo prazo. OBJETIVO: Caracterizar o perfil de pacientes candidatos à cirurgia bariátrica a partir da avaliação psicológica pré-operatória. MÉTODOS: Estudo observacional transversal. Um total de 414 avaliações psicológicas pré-operatórias de uma clínica particular do sul de Santa Catarina foram analisadas, dos anos de 2011 a 2016. RESULTADOS: Dos prontuários avaliados, 311 (75,1%) eram do sexo feminino, a mediana de idade foi de 36 anos. O uso abusivo de substâncias foi encontrado em 66 (15,9%) prontuários, sendo a nicotina (8,7%) e o álcool (4,1%) as substâncias mais usadas. 26,6% dos pacientes referiram ansiedade e 5,1% referiram depressão. 17,4% dos pacientes apresentavam escore elevado no teste de BITE (Teste de investigação bulímica de Edimburgo) – caracterizando a presença de bulimia e 25,6 % apresentaram padrão alimentar não usual, sem caracterizar a bulimia. As mulheres apresentaram mais chances de ter bulimia (OR= 2,642; IC95%:1,288 a 5,420; p=0,007). A chance maior de ter bulimia também aconteceu em quem tinha diagnóstico atual de doença psiquiátrica (OR=2,351; IC=1,275 a 4,335; p=0,006). Ainda, quem possuía ansiedade também teve risco maior para apresentar bulimia (OR=1,971; IC=1,70 a 3,631; p=0,029). CONCLUSÃO: A maioria dos pacientes candidatos à cirurgia bariátrica são mulheres, o sexo feminino também tem mais chances de ter bulimia.
