Dermatological manifestations in patients of a psychiatry ambulatory

Background: psychiatric illness can often lead to dermatological changes as well as skin disorders can cause depressive and anxiety symptoms. Objective: to evaluate the prevalence of skin disorders in patients attended at an outpatient psychiatric school in Tubarão, Santa Catarina. Method: a cross-sectional, observational study which 73 subjects from the psychiatric outpatient clinic were interviewed using questionnaires, and medical records were subsequently analyzed to obtain current psychiatric information. Result: skin disorders were reported by 53.4% of the patients, being dyschromias, allergies, acne and psoriasis the most frequent. In the association between gender and the presence of dermatoses, the female gender was described in 60.3% (p < 0.02) of patients with skin manifestations. Among the participants with dermatological manifestations, 41.0% reported a significant change in humor and self-esteem due to the skin disease in question, and 35.9% reported little change in these variables. Depressive, anxiety and humor disorders had a higher prevalence of skin diseases with 53.2%, 52.4% and 53.9% respectively. Study limitations: the dermatoses were self-reported by the patients, since a dermatological examination was not performed, as the group size was small, and it could reduce the statistical significance in the analysis of some variables. Conclusion: the majority of patients with psychiatric disorders reported dermatological manifestations, and women were associated with more statistically significant skin diseases. Objetivos: avaliar a prevalência de queixas dermatológicas nos pacientes atendidos em um ambulatório-escola de psiquiatria em Tubarão, Santa Catarina. Métodos: estudo observacional do tipo transversal, em que foram entrevistados 73 pacientes do ambulatório de psiquiatria por meio de questionários e, posteriormente, foram analisados os prontuários médicos para obtenção de informações psiquiátricas atuais. Resultados: alterações dermatológicas foram referidas por 53,4% dos pacientes, sendo discromias, alergias, acne e psoríase as mais frequentes. Na associação entre gênero e presença de dermatoses, o gênero feminino foi descrito em 60,3% (p < 0,02) dos pacientes com alterações de pele. Dentre os participantes com alterações dermatológicas, 41,0% referiram muita alteração de humor e autoestima devido à doença de pele em questão e 35,9% relataram pouca alteração nessas variáveis. Os transtornos depressivos, de ansiedade e de humor tiveram uma maior prevalência de doenças de pele com 53,2%, 52,4% e 53,9% respectivamente. Limitações do estudo: inerentes ao fato de as dermatoses terem sido autorreferidas pelos pacientes, uma vez que não foi realizado exame físico dermatológico, assim como o tamanho da amostra ter sido pequena, podendo diminuir a significância estatística no cruzamentos de algumas variáveis. Conclusões: verificou-se que a maioria dos pacientes com patologias psiquiátricas referiu manifestações dermatológicas, sendo que as mulheres foram associadas a doenças de pele com maior significância estatística.
