Study of risk factors for the development of retinopathy of prematurity in premature new born with very low birth weight

Objective: to evaluate the risk factors for the occurrence of retinopathy of prematurity (ROP) in premature newborns (Gestational Age ≤ 32 weeks) with birth weight (BW) ≤ 1500g that attended the ophthalmology clinic of HRSJ between January of 2012 and November of 2017. Method: Case study, retrospective and observational. The study included patients with GA ≤ 32 weeks and very low weight who were seen at the HRSJ ophthalmology outpatient clinic between January 1, 2012 and November 30, 2017. All preterm infants who died prior to the first ophthalmologic exam were excluded from the study. The sample was tabulated directly in the SPSS program and the categorical variables were compared by the Fisher and Qui-Square exact tests, with significance level p <0.05. The odds ratio (OR) for retinopathy, and their respective confidence intervals (95% CI) were calculated. After the bivariate analysis, the variables with statistical significance (p <0.05) were submitted to multivariate analysis Results: A higher ROP stage 1 level was observed among the NBs studied. The presence of ROP was greater in the BW group <1000 grams (80.00%), and less than 27 weeks (75.00%). With logistic regression, sepsis and days of oxygen therapy remained statistically significant. Conclusion: The majority of the NBs presented ROP stage 1. With the multivariate analysis, days of oxygen therapy (OR 3.042/IC 1.167-7.930) and sepsis (OR 2.738/IC 1.198-6.256), remained as risk factors, indicating a true association with the development of ROP.Objetivo: Avaliar a os fatores de risco para a ocorrência da retinopatia da prematuridade (ROP) em recém-nascidos (RN) prematuros (Idade Gestacional ≤ 32 semanas) e com peso ao nascimento (PN) ≤ 1500g atendidos no ambulatório de oftalmologia do HRSJ entre janeiro de 2012 e novembro de 2017. Método: Estudo caso controle, retrospectivo e observacional. Foram incluídos no estudo pacientes com IG ≤ 32 semanas e de muito baixo peso que foram atendidos no ambulatório de oftalmologia do HRSJ entre 1 de janeiro de 2012 a 30 de novembro de 2017. Foram excluídos do estudo todos os RNs prematuros que foram a óbito antes da realização do primeiro exame oftalmológico. A amostra foi tabulada diretamente no programa SPSS e as variáveis categóricas foram comparadas pelos testes exato de Fisher e Qui-Quadrado, com nível de significância p<0,05. Foram calculadas as razões de chance (OR) de retinopatia, e os seus respectivos intervalos de confiança (IC95%). Após a análise bivariada, as variáveis com significância estatística (p<0.05) foram submetidas a análise multivariada. Resultados: Observou-se um maior índice de ROP estágio 1 entre os RNs estudados. A presença da ROP foi maior no grupo com PN < 1000 gramas (80,00%), e com IG menor que 27 semanas (75,00%). Com a regressão logística, obtiveram significância estatística sepse e dias de oxigenioterapia. Conclusão: A maioria dos RNs apresentaram ROP estágio 1. Com a realização da regressão logística mantiveram-se como fatores de risco dias de oxigenioterapia (OR 3.042/IC 1.167-7.930) e sepse (OR 2.738/IC 1.198-6.256), indicando uma associação verdadeira com o desenvolvimento da ROP
